Alternating Fever and Chills

How Chinese medicine differentiates alternating fever and chills, the two patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Alternating fever and chills lasting more than a few days, or with severe pain, always needs medical evaluation before assuming it is a simple pattern.
General Acupuncture Points

Acupuncture points traditionally used for alternating fever and chills — 5 key points, used across the patterns below rather than tied to just one.

PointKey action
G.B.-34Harmonizes the Gallbladder channel, relieves rib-side fullness.
LR-3Spreads Liver-Qi, calms irritability.
L.I.-4Releases the exterior, regulates Defensive Qi.
G.B.-20Dispels Wind, clears the head.
BL-18Clears Liver-Heat, regulates Liver-Qi.
The Two Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Lesser Yang Stage

Clinical manifestations: Alternating waves of chills and fever, bitter taste in the mouth, fullness and tightness along the ribcage, nausea and loss of appetite, irritability and a desire to be left alone.
Tongue: Normal or slightly red body, red sides possible, thin white or thin yellow coating. Pulse: Wiry and fine, like a taut guitar string.
Treatment principle: Harmonize the Shao Yang, soothe the Liver and regulate Qi, restoring smooth Qi flow through the half-exterior half-interior gateway.
Note: no moxa, even technique throughout given the mixed half-exterior half-interior nature. This is a Six-Stages concept rather than a Zang-Fu organ pattern, and no deck on this site has a Shao-Yang/Lesser-Yang-stage slide, so no pattern link is given here.

2. Liver and Gallbladder Damp-Heat View full pattern →

Clinical manifestations: Pain or distension below the ribs especially on the right side, persistent bitter taste in the mouth, nausea or aversion to greasy food, dark yellow scanty urine, a heavier more persistent quality to the alternating temperature sensation.
Tongue: Red body with red sides and a thick yellow greasy coating. Pulse: Wiry, slippery and rapid.
Treatment principle: Drain excess Fire from the Liver and Gallbladder, clear Damp-Heat from the Lower Burner, purging stuck Dampness and Heat to restore smooth Qi circulation.
Note: no moxa. Direct match with this site's own Damp-Heat-in-the-Gall-Bladder-and-Liver slide — its core presentation of rib pain, bitter taste, and dark urine is this pattern's own picture.
