Hypochondrial pain, fullness and distension, nausea, vomiting, inability to digest fats, yellow complexion, scanty and dark yellow urine, fever, thirst without desire to drink, bitter taste, dizziness, tinnitus, irritability, feeling of heaviness of the body, numbness of the limbs, swelling of the feet, loose stools or constipation, alternation of hot and cold feeling, yellow sclera, feeling of heat.

Principle: resolve Dampness, clear Heat in the Gall Bladder, stimulate the smooth flow of Liver-Qi

Combination Points (and notes)

• A candid clinical observation worth repeating to students: Chinese texts always emphasize Damp-Heat when describing Gall Bladder patterns, but many Western patients present with Dampness alone, without much Heat — don't force a Heat diagnosis onto a presentation that's really just Dampness in the Gall Bladder.
• Gall Bladder stones are described as an extreme, most-substantial form of Dampness and Phlegm, formed over a long period under the 'steaming and brewing' action of Heat — worth knowing as the Chinese-medicine framing of cholelithiasis when this pattern is suspected clinically.

Clinical Presentation

Dizziness, blurred vision, floaters, nervousness, timidity, propensity to being easily startled, lack of courage and initiative, indecision, sighing, waking up early in the morning, restless dreams.

Treatment

Principle: tonify and warm the Gall Bladder, tonify Liver-Qi

Combination Points (and notes)

• This is less a clinical 'pattern' than a description of character, explicitly: timidity, lack of courage and indecision are the defining feature, not a set of somatic symptoms — and it may equally originate from childhood family dynamics rather than any organ pathology, in which case it should not be 'diagnosed' as this pattern at all.
• Zhang's 'Classic of Categories' (1624) puts it well: 'The Liver stores Blood and Blood houses the Ethereal Soul. If the Liver is deficient there is fear; if in excess there is anger' — Gall Bladder deficient is essentially Liver-Qi deficiency (often with an underlying Liver-Blood deficiency) expressed as fear, timidity and indecision, since the Gall Bladder is the Yang aspect of the Liver and Liver-Yang itself is said to be never deficient, only ever in excess.

• A clean one-line differentiator from Damp-Heat in the Gall Bladder alone: the presence of genital symptoms (excessive yellow vaginal discharge, genital papular skin rashes and itching, swelling and heat of the scrotum) — these appear because the Liver channel itself, not just the Gall Bladder, is now involved.
• Emotional strain (long-standing anger causing Liver-Qi stagnation that implodes into Heat) is noted as playing a larger role here than in Damp-Heat in the Gall Bladder alone — worth asking specifically about anger and frustration when genital/urinary symptoms accompany the hypochondrial picture.
