Flank Pain

How Chinese medicine differentiates flank pain, the four patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Flank pain with fever, jaundice, or unexplained weight loss needs medical evaluation before assuming it is a simple pattern.
General Acupuncture Points

Acupuncture points traditionally used for flank pain — 6 key points, used across the patterns below rather than tied to just one.

PointKey action
LR-14Front-Mu point of the Liver, directly soothes Liver-Qi and releases chest and flank tension.
G.B.-34He-Sea point of the Gall Bladder, harmonizes Liver and Gall Bladder.
LR-3Yuan-source point, powerfully unbinds stagnant Qi.
BL-17Influential point for Blood, invigorates Blood and breaks stasis.
SP-10Sea of Blood, promotes circulation and alleviates stasis pain.
SP-6Nourishes Yin and Blood across all three Yin channels.
BL-18Back-Shu point of the Liver, directly tonifies Liver-Yin.
BL-23Strengthens Kidney-Yin to support the Liver.
The Four Patterns

1. Liver-Qi Stagnation View full pattern →

Clinical manifestations: Distending, wandering pain in the ribs and flanks, pain triggered or worsened by emotional stress, frequent sighing and chest tightness, irritability.
Tongue: Normal or slightly red body with red or slightly purple sides, thin white coating. Pulse: Wiry, especially at left middle position.
Treatment principle: Course the Liver and resolve constraint, move Qi and alleviate pain.
Note: no moxa. Direct match with this site's own Liver-Qi-stagnation slide.

2. Liver-Blood Stagnation View full pattern →

Clinical manifestations: Fixed stabbing pain in one spot, dark menstrual blood with clots in women, purple or dark lips and nails, dry rough or scaly skin, pain worse with pressure.
Tongue: Dark purple body with purple stasis spots on the sides, distended dark sublingual veins, thin white coat. Pulse: Wiry and choppy, like a knife scraping bamboo.
Treatment principle: Invigorate Blood and dispel stasis, open the chest and disperse stagnation.
Note: no moxa. Direct match with this site's own Liver-Blood-stasis slide — its title corresponds almost exactly.

3. Liver and Gallbladder Damp-Heat View full pattern →

Clinical manifestations: Burning or distending pain especially on the right side, bitter taste in the mouth, nausea or aversion to greasy food, dark yellow scanty urine, irritability.
Tongue: Red body with redder sides and thick yellow greasy coating. Pulse: Wiry, slippery and rapid.
Treatment principle: Drain excess Fire from the Liver and Gall Bladder, clear Damp-Heat from the Lower Burner.
Note: no moxa. Direct match with this site's own Damp-Heat-in-the-Gall-Bladder-and-Liver slide.

4. Liver-Yin Deficiency View full pattern →

Clinical manifestations: Dull burning ache below the ribs, dry gritty eyes and dry mouth, night sweats and afternoon low-grade fever, irritability.
Tongue: Red, thin body with redder sides, dry surface, little or no coating, may appear mirror-like or peeled. Pulse: Wiry, fine and rapid.
Treatment principle: Nourish Liver and Kidney Yin, soothe the Liver and regulate Qi.
Note: no moxa given the Yin deficiency. Direct match with this site's own Liver-Yin-deficiency slide.
