Globus Sensation (Plum Pit Throat)

How Chinese medicine differentiates the sensation of a lump in the throat, the six patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Actual difficulty swallowing food or liquid, unintended weight loss, or a lump that keeps growing needs medical evaluation to rule out a structural cause before assuming it is a simple Qi-Phlegm pattern.
General Acupuncture Points

Acupuncture points traditionally used for globus sensation — 7 key points, used across the patterns below rather than tied to just one.

PointKey action
REN-22Directly opens the throat, promotes Qi descent, and transforms phlegm.
P-6Regulates Qi in the chest, eases emotional constraint.
ST-40Key point to transform phlegm and dampness.
ST-36Strengthens the Spleen to prevent phlegm formation.
LR-3Subdues Liver Qi stagnation and eases throat constriction.
L.I.-11Clears heat and cools the body, settles phlegm-heat.
SP-10Invigorates blood and dispels stasis.
The Six Patterns

1. Qi-Phlegm

Clinical manifestations: Sensation of a lump in the throat that cannot be swallowed or coughed up, abdominal bloating, nausea, frequent sighing, worse with emotional stress, chest fullness.
Tongue: Normal pink-red body, possibly slightly swollen, white greasy or slippery coating. Pulse: Wiry and slippery.
Treatment principle: Move Qi and dissipate nodules, descend Qi, resolve Phlegm.
Note: no moxa. No deck on this site combines Liver-Qi stagnation with Phlegm into one slide, so no pattern link is given here.

2. Liver Qi Stagnation View full pattern →

Clinical manifestations: Symptoms worsen with emotional stress, frequent sighing, irritability or low mood, chest oppression and rib distension, sensation comes and goes with mood fluctuations.
Tongue: Normal or slightly dark body with red or purplish sides, thin white coat. Pulse: Wiry, taut like a guitar string.
Treatment principle: Course the Liver and resolve constraint.
Note: no moxa. Direct match on the core distension/irritability/sighing picture.

3. Obstruction of the Spleen by Dampness with Liver Qi Stagnation View full pattern →

Clinical manifestations: Lump sensation worsens after eating or with emotional stress, abdominal bloating, sticky/incomplete bowel movements or loose stools, heaviness of body and limbs, irritability or low mood.
Tongue: Pale and swollen with teeth marks, white greasy coating, excessively moist. Pulse: Slippery and wiry.
Treatment principle: Harmonize the Shao Yang, dry Dampness, transform Phlegm.
Note: no moxa except tonifying points. Direct name match with this site's own Obstruction-of-Spleen-by-Dampness-with-stagnation-of-Liver-Qi slide.

4. Rebellious Stomach Qi View full pattern →

Clinical manifestations: Lump sensation worsens after eating or with emotional stress, frequent belching or burping, nausea, sensation of Qi rising from stomach to throat, loss of appetite.
Tongue: Normal body colour with a white coating. Pulse: Wiry, often slippery, full and taut at the right middle.
Treatment principle: Descend Qi, resolve Phlegm, tonify Qi.
Note: no moxa. Direct match — nausea, belching, vomiting, and difficulty swallowing are named on this site's Stomach-Qi-rebelling-upwards slide.

5. Phlegm-Heat

Clinical manifestations: Sensation of a hot, sticky lump in the throat, thick yellow phlegm, thirst for cold drinks, irritability, red flushed face, chest fullness.
Tongue: Red body, swollen with a thick yellow greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clear Heat and transform Phlegm, clear Liver and Gallbladder Heat.
Note: no moxa. No deck on this site combines Liver/Gallbladder Heat with Phlegm specifically in the throat, so no pattern link is given here.

6. Qi and Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing pain in throat or chest, sensation of a hard, immovable lump, chest oppression and rib distension, chronic presentation.
Tongue: Dark purple body with stasis spots, distended sublingual veins, thin white coat. Pulse: Wiry and choppy.
Treatment principle: Invigorate Blood and dispel stasis, move Qi and alleviate pain.
Note: no moxa. Matches this site's Liver-Blood stasis slide — a dark purple tongue with stasis spots is the same discoloration marker named there.
