Intestinal Obstruction

How Chinese medicine differentiates intestinal obstruction — distinct from Constipation and Abdominal Fullness already covered — the five patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Intestinal obstruction is a surgical emergency — severe abdominal pain and distension with no passage of gas or stool needs immediate medical care, not acupuncture first; the patterns below are for understanding the TCM picture alongside that care, never as a substitute for it.
General Acupuncture Points

Acupuncture points traditionally used for intestinal obstruction — 11 key points, used across the patterns below rather than tied to just one.

The Five Patterns

1. Qi Stagnation View full pattern →

Clinical manifestations: Intermittent distending pain that shifts around the abdomen, temporary relief after passing gas, loud gurgling sounds, symptoms worsen with stress or frustration.
Tongue: Pale or slightly dusky body with thin white coating. Pulse: Wiry, like a taut guitar string.
Treatment principle: Move Qi, descend Qi, soothe the Liver and regulate Qi.
Note: no moxa. Direct exact-name match with this site's own Qi-Stagnation-in-the-Large-Intestine slide.

2. Bright Yang Fire in Stomach and Intestines View full pattern →

Clinical manifestations: High fever worse in the afternoon, severe abdominal distension painful to touch, complete constipation with dry hard stools, intense thirst for cold drinks, foul-smelling breath and gas, dark scanty urine.
Tongue: Red body with prickly thorns, thick yellow dry coating possibly cracked. Pulse: Deep, forceful, rapid, slippery.
Treatment principle: Vigorously purge Heat accumulation, promote bowel movement, move Qi and relieve distension.
Note: no moxa. Direct match with this site's own Heat-obstructing-the-Large-Intestine slide, itself badged Emergency — high/tidal fever with delirium, abdominal pain and constipation is its own exact diagnostic tip.

3. Damp-Heat in the Large Intestine View full pattern →

Clinical manifestations: Foul-smelling, sticky diarrhoea or loose stools, burning sensation at the anus, heaviness in the abdomen and limbs, nausea and poor appetite, urgent straining with incomplete evacuation.
Tongue: Red body with thick yellow greasy coating, thickest at the centre and root. Pulse: Slippery and rapid.
Treatment principle: Clear Heat and dry Dampness, invigorate Blood and move Qi, resolve toxicity.
Note: no moxa. Direct exact-name match with this site's own Damp-Heat-in-the-Large-Intestine slide.

4. Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing abdominal pain, pain worse at night and with pressure, dark tarry or blood-streaked stools, palpable hard mass in the abdomen, dark purplish lips or complexion.
Tongue: Dark purple body with stasis spots, distended sublingual veins, thin white coat. Pulse: Choppy and wiry, often rapid.
Treatment principle: Invigorate Blood and dispel stasis, move Qi and alleviate pain, open the chest and disperse stagnation.
Note: no moxa except on ST-36. Direct match with this site's own Liver-Blood-stasis slide via the well-established dark/purple stasis-spot-tongue marker, plus a palpable hard mass.

5. Cold Invading the Large Intestine View full pattern →

Clinical manifestations: Sudden cramping lower abdominal pain, cold sensation in the abdomen relieved by warmth, preference for warm drinks with no thirst, cold hands and feet, loud gurgling bowel sounds.
Tongue: Pale body, thick white slippery moist coat. Pulse: Deep and tight, especially at the right middle position.
Treatment principle: Warm the interior and dispel Cold, unblock the bowels, alleviate pain.
Note: moxa central to this pattern (REN-8 and REN-4 both needled/moxa'd to warm the interior). Direct exact-name match with this site's own Cold-invading-the-Large-Intestine slide.
