Scleroderma

How Chinese medicine differentiates scleroderma — hard, tightened skin from an autoimmune process — into the six patterns you'll actually meet in practice, and the points for each. Supportive care alongside rheumatology management, not a substitute for it. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Scleroderma needs ongoing rheumatology care, especially to monitor for internal organ involvement; acupuncture here is supportive of the underlying pattern, not a substitute for that care.

The Six Patterns

1. Painful Obstruction With Wind-Cold-Damp

Clinical manifestations: Skin tightening with a cold sensation, joint/muscle pain worse in cold/damp weather, stiffness and limited range of motion, heaviness or numbness in the limbs, pain relieved by warmth and gentle movement.
Tongue: Pale, slightly swollen body with teeth marks, white greasy coating, moist surface. Pulse: Wiry and tight, or slow, may feel soggy if Dampness predominates.
Treatment principle: Warm the channels and disperse Cold, expel Wind, Cold and Dampness.
Note: moxa applicable. This site has no organ-nonspecific Wind-Cold-Damp Bi-syndrome slide, a gap confirmed absent many times over — no pattern link is given here; points still link fine individually.

2. Painful Obstruction Due to Damp Heat in Channels

Clinical manifestations: Skin redness and warmth, purple-red hue, pain worsened by heat and eased by cold, heavy aching limbs, fever or bodily heat sensation, thirst, dark scanty urine.
Tongue: Red body, possibly swollen, yellow greasy coating. Pulse: Slippery and rapid, possibly soggy or wiry at the Guan position.
Treatment principle: Clear Heat and drain Dampness, unblock channels and collaterals.
Note: no moxa. This site has no organ-nonspecific Damp-Heat-in-channels Bi-syndrome slide, a gap confirmed absent many times over — no pattern link is given here; points still link fine individually.

3. Qi and Blood Deficiency

Clinical manifestations: Thin, atrophic, papery skin, pale or sallow complexion, muscle wasting and weakness, severe fatigue, dizziness, spontaneous sweating, thirst.
Tongue: Pale body, may be thin or have teeth marks, thin white coating. Pulse: Fine and weak, may feel empty or slow.
Treatment principle: Tonify Qi and nourish Blood, warm channels and invigorate circulation.
Note: moxa applicable throughout. This site has no combined Qi-and-Blood-deficiency slide, a gap confirmed absent many times over — no pattern link is given here; points still link fine individually.

4. Blood Stagnation View full pattern →

Clinical manifestations: Hard, leathery skin that cannot be pinched, dark or purplish discoloration, fixed stabbing joint/muscle pain, stiffness and limited range of motion.
Tongue: Dark purple body with stasis spots, distended sublingual veins, thin white or greasy coating. Pulse: Choppy, or wiry if Qi stagnation is prominent.
Treatment principle: Invigorate Blood and dispel stasis, unblock channels.
Note: no moxa. Direct match with this site's own Liver-Blood-stasis slide.

5. Phlegm in the Channels, Joints and Muscles

Clinical manifestations: Fixed, stabbing joint/muscle pain, hard non-pitting swelling or subcutaneous lumps, dark/purplish discoloration, thick white greasy tongue coating, heaviness or numbness in the limbs.
Tongue: Purple body with stasis spots, white greasy coating, distended sublingual veins. Pulse: Wiry and choppy, or wiry and slippery if Phlegm dominates.
Treatment principle: Transform Phlegm and disperse Wind-Cold, invigorate Blood and unblock collaterals.
Note: no moxa. This site has no Phlegm-in-the-channels/joints/muscles slide on any deck, a gap confirmed absent many times over — no pattern link is given here; points still link fine individually.

6. Kidney and Spleen Yang Deficiency View full pattern →

Clinical manifestations: Hard but paper-thin skin, cold limbs especially below the knees, chronic loose stools/early-morning diarrhea, sore and cold lower back/knees, severe fatigue and lack of energy.
