Kidney-Yang deficiency

Kidney PatternEmpty (Deficiency) Pattern

Clinical Presentation

Lower backache, dizziness, tinnitus, cold and weak knees, sensation of cold in the lower back, feeling cold, weak legs, bright-white complexion, tiredness, lassitude, abundant clear urination, urination at night, apathy, oedema of the legs, infertility in women, loose stools, depression, impotence, premature ejaculation, low sperm count, decreased libido.

Tongue: Pale and wet. Pulse: Deep-Weak.
Important symptoms: backache, feeling of cold, abundant clear urination, Pale tongue, Deep pulse.
Arises from: Spleen-Yang deficiency is the most common precursor of this pattern. Retention of Dampness (resulting from Spleen deficiency) over a long period of time will eventually affect the Kidneys by obstructing the movement of fluids and therefore leading to deficiency of Kidney-Yang. A deficiency of Kidney-Yang may also evolve from Kidney-Yin deficiency, in which case both Yin and Yang will be deficient (although with a predominance of Yang deficiency).
Typically develops into: Kidney-Yang deficiency may induce a Yang deficiency of various organs and especially the Spleen, Stomach, Lungs and Heart. Indeed, in chronic conditions, a long-standing deficiency of Kidney-Yang will always affect one or more of the above organs.
Easy clinical tip: Backache, a feeling of cold and a Weak Kidney pulse are enough to diagnose Kidney-Yang deficiency.

Treatment

Principle: tonify and warm the Kidneys, strengthen the Fire of the Gate of Life

BL-23 Tonifies Kidney-Yang.
Du-4 Strengthens the Fire of the Gate of Life. Moxa is applicable.
Ren-4 With moxa, tonifies Kidney-Yang and the Original Qi.
Ren-6 With moxa, tonifies Kidney-Yang.
KI-3 Tonifies the Kidneys.
KI-7 Specific to tonify Kidney-Yang.
BL-52 Tonifies the Kidneys and, in particular, their mental aspect β€” the Will-power (Zhi).
Jinggong Extra point (0.5 cun lateral to BL-52) β€” tonifies Kidney-Yang and warms the Essence.

Combination Points (and notes)

  • This is the base Kidney-Yang tonification set (BL-23, Du-4, Ren-4, Ren-6, KI-3, KI-7) that recurs, with additions, in Kidney-Qi not Firm, Kidneys failing to receive Qi and Kidney- and Spleen-Yang deficiency β€” worth learning as a single unit before the more complex patterns.
  • BL-52 is specifically included for the mental/Will-power dimension of Kidney-Yang deficiency (apathy, depression) β€” not just the physical cold and backache.

Kidney-Yin deficiency

Kidney PatternEmpty (Deficiency) Pattern

Clinical Presentation

Dizziness, tinnitus, vertigo, poor memory, hardness of hearing, night-sweating, dry mouth and throat at night, lower backache, ache in bones, nocturnal emissions, constipation, dark scanty urine, infertility, premature ejaculation, tiredness, lassitude, depression, slight anxiety. With pronounced Empty-Heat: five-palm heat, feeling of heat in the evening, malar flush, menopausal hot flushes, thirst with desire to drink in small sips, pronounced anxiety in the evening, nocturnal emissions with dreams.

Tongue: Normal-coloured without coating. With Empty-Heat: Red without coating, in severe cases also cracked. Pulse: Floating-Empty. With Empty-Heat: Floating-Empty and slightly Rapid.
Important symptoms: backache, night sweating.
Arises from: A deficiency of Liver-Yin may lead to Kidney-Yin deficiency as the Liver and Kidneys share a common root. However, deficiency of Yin of the Heart or Lungs may also lead to Kidney-Yin deficiency. In women, a long-standing deficiency of Liver-Blood may also lead to Kidney-Yin deficiency. A deficiency of Kidney-Yin may also evolve from Kidney-Yang deficiency, in which case both Yin and Yang will be deficient (although with a predominance of Yin deficiency).
Typically develops into: Kidney-Yin deficiency may lead to Yin deficiency of various organs and particularly Liver, Heart, Lungs and Stomach.
Easy clinical tip: Even just backache and night sweating are enough to diagnose Kidney-Yin deficiency.

Treatment

Principle: nourish Kidney-Yin

Ren-4 Without moxa, tonifies Kidney-Yin and Kidney-Essence (with moxa it can tonify Kidney-Yang instead).
KI-3 Tonifies the Kidneys.
KI-6 Specific to tonify Kidney-Yin and benefits the throat β€” particularly indicated for dry mouth at night.
KI-10 Specific to tonify Kidney-Yin.
KI-9 Tonifies Kidney-Yin β€” particularly useful for anxiety and emotional tension of Kidney origin.
SP-6 Tonifies Liver- and Kidney-Yin and calms the Mind.
Ren-7 Nourishes Yin.
LU-7 In combination with KI-6, opens the Directing Vessel (Ren Mai) and nourishes Kidney-Yin.

Combination Points (and notes)

  • Ren-4's dual behaviour is a good teaching point in itself: without moxa it tonifies Kidney-Yin, but with moxa the very same point tonifies Kidney-Yang instead β€” technique, not just point selection, determines the outcome.
  • A completely peeled, dry tongue with a deep midline crack always indicates severe Kidney-Yin deficiency β€” a reliable tongue signature to check for independent of the presenting complaint.

Kidney-Qi not Firm

Kidney PatternEmpty (Deficiency) Pattern

Clinical Presentation

Soreness and weakness of the lower back, weak knees, clear frequent urination, weak-stream urination, abundant urination, dribbling after urination, incontinence of urine, enuresis, urination at night, nocturnal emissions without dreams, premature ejaculation, spermatorrhoea, tiredness, feeling cold, cold limbs; in women, prolapse of uterus, chronic white vaginal discharge, a dragging-down feeling in the lower abdomen, recurrent miscarriage.

Tongue: Pale. Pulse: Deep-Weak, especially in the Rear positions.
Important symptoms: dribbling after urination, chronic vaginal discharge, backache.
Arises from: Kidney-Yang deficiency is a precursor of this pattern. A long-standing, chronic deficiency of Spleen-Yang may also lead to Kidney-Qi not Firm.
Typically develops into: Kidney-Qi not Firm may easily lead to full-blown Kidney-Yang deficiency.
Easy clinical tip: Dribbling after urination, chronic vaginal discharge and backache are enough to diagnose Kidney-Qi not Firm.

Treatment

Principle: reinforce and stabilize Kidney-Qi, raise Qi, tonify Kidney-Yang

BL-23 Tonifies Kidney-Yang and firms Qi.
Du-4 Tonifies Kidney-Yang and the Fire of the Gate of Life β€” important to stop incontinence, enuresis and excessive urination.
KI-3 Tonifies the Kidneys.
BL-52 Tonifies Kidney-Yang and strengthens the Will-power.
Ren-4 With moxa, tonifies Kidney-Yang and the Original Qi.
Jinggong Tonifies Kidney-Yang and firms the Sperm Gate.
Ren-6 Tonifies and firms Qi.
Du-20 Raises sinking Qi.
KI-13 Strengthens the Sperm Gate and tonifies the Kidneys.
BL-32 Firms Qi in the sexual organs.

Combination Points (and notes)

  • This pattern is memorably summarized as everything 'leaking out' β€” frequent urination, incontinence, premature ejaculation, seminal emissions, urethral discharge β€” a single unifying image worth teaching before the point-by-point detail.
  • Du-20 (raising sinking Qi) and Jinggong (firming the Sperm Gate) show the two directions this pattern is treated from: lifting what has sunk, and closing what has become unfirm.

Kidneys failing to receive Qi

Kidney PatternEmpty (Deficiency) Pattern

Clinical Presentation

Shortness of breath on exertion, rapid and weak breathing, difficulty in inhaling, chronic cough and/or asthma, spontaneous sweating, cold limbs, cold limbs after sweating, swelling of the face, thin body, mental listlessness, clear urination during an asthma attack, lower backache, dizziness, tinnitus.

Tongue: Pale. Pulse: Deep-Weak-Tight.
Important symptoms: shortness of breath on exertion, lower backache, clear urination.
Arises from: A deficiency of Kidney-Yang may lead to this pattern. A deficiency of Lung-Qi may also lead to this pattern if it is very prolonged.
Typically develops into: The pattern of Kidneys failing to receive Qi may lead to the formation of Phlegm, which, itself, would aggravate the breathlessness.
Easy clinical tip: Shortness of breath on exertion, lower backache and clear urination are enough to diagnose Kidneys failing to receive Qi.

Treatment

Principle: tonify and warm the Kidneys, stimulate the Kidney's receiving of Qi, stimulate the descending of Lung-Qi

KI-7 Strengthens the Kidney's receiving of Qi.
KI-3 Tonifies the Kidneys.
LU-7 In combination with KI-6, opens the Directing Vessel, stimulates the descending of Lung-Qi and the Kidney's reception of Qi, and benefits the throat.
KI-6 In combination with LU-7, opens the Directing Vessel and stimulates the Kidney's reception of Qi.
ST-36 Tonifies Qi in general β€” important in chronic conditions.
BL-23 With Du-4, tonifies Kidney-Yang.
Du-4 With BL-23, tonifies Kidney-Yang.
Ren-6 With moxa, tonifies Kidney-Yang and draws Qi down to the abdomen.
Ren-17 Tonifies Qi and stimulates the descending of Lung-Qi.
KI-25 Important local chest point of the Kidney channel β€” stimulates the Kidney's reception of Qi and improves breathing.
Du-12 With BL-13, tonifies Lung-Qi β€” important in chronic conditions.

Combination Points (and notes)

  • A clean differentiator from ordinary Kidney-Yang deficiency: here the focus is squarely on breathing β€” chronic asthma, breathlessness, difficulty inhaling β€” rather than backache and cold being the leading complaint.
  • LU-7 + KI-6 (opening the Ren Mai) recurs here exactly as in Lung-Yin deficiency, but for a different purpose β€” restoring the Kidney's grasp of Qi rather than nourishing Yin β€” a good example of the same point combination serving different clinical goals.

Kidney-Essence deficiency

Kidney PatternEmpty (Deficiency) Pattern

Clinical Presentation

In children: poor bone development, late closure of the fontanelle, deafness, mental dullness or retardation. In adults: softening of bones, weakness of knees and legs, poor memory, loose teeth, falling hair or premature greying of hair, weakness of sexual activity, lower backache, infertility, sterility, primary amenorrhoea, dizziness, tinnitus, deafness, blurred vision, absentmindedness, decreased mental sharpness.

Tongue: Without coating if against a background of Kidney-Yin deficiency; Pale if against a background of Kidney-Yang deficiency. Pulse: Floating-Empty or Leather.
Important symptoms: children: poor bone development; adults: weak knees, falling hair, weak sexual activity.
Arises from: Both Kidney-Yin and Kidney-Yang deficiency may lead to Kidney-Essence deficiency.
Typically develops into: Kidney-Essence deficiency may lead to Kidney-Yin or Kidney-Yang deficiency. In women, it may also lead to Liver-Blood deficiency as the Essence plays a role in making Blood.
Easy clinical tip: Poor bone development in children; weak knees, falling hair and weak sexual activity in adults are enough to diagnose Kidney-Essence deficiency.

Treatment

Principle: nourish the Essence, tonify the Kidneys

KI-3 Tonifies Kidney-Yin and Essence.
KI-6 Tonifies Kidney-Yin.
Ren-4 With KI-13, tonifies the Essence.
KI-13 With Ren-4, tonifies the Essence.
BL-23 Tonifies the Kidneys.
Du-4 Tonifies the Yang aspect of the Essence β€” used only if the deficiency occurs on a background of pronounced Yang deficiency.
G.B.-39 Tonifies the Bone Marrow.
Du-20 Stimulates the Marrow to fill the Brain.
BL-15 Tonifies the Heart to house the Mind, hence tonifies the brain.
BL-11 Nourishes the Bones.
Du-17 With Du-16, a point of the Sea of Marrow β€” stimulates the nourishment of the Brain by Marrow.
Du-16 With Du-17, stimulates the nourishment of the Brain by Marrow.

Combination Points (and notes)

  • Essence-deficiency symptoms group into three memorable areas: Brain (poor memory, dizziness), Bones (weak bones), and Hair (loss of hair, premature greying) β€” a useful three-part checklist when screening for this pattern.
  • Du-4 is conditional, not routine β€” used only when the Essence deficiency sits on a background of pronounced Yang deficiency, and moxa generally is skipped if there is marked Yin deficiency with Empty-Heat instead.

Kidney-Yang deficiency, Water overflowing

Kidney PatternEmpty (Deficiency) Pattern

Clinical Presentation

Oedema, especially of the legs and ankles, cold feeling in legs and back, fullness and distension of the abdomen, soreness of lower back, feeling cold, scanty clear urination. Water overflowing to the Heart adds palpitations, breathlessness, cold hands. Water overflowing to the Lungs adds thin watery frothy sputum, cough, asthma and breathlessness on exertion.

Tongue: Pale, Swollen and wet with a white coating. Pulse: Deep-Weak-Slow.
Important symptoms: oedema of ankles, Deep-Weak pulse, Pale-Swollen tongue.
Arises from: Chronic, long-standing retention of Dampness which interferes with the Kidney function of transformation of fluids may lead to this pattern. This pattern may be transmitted from long-term Spleen-Yang deficiency, especially when there is also Dampness. In the case of Water overflowing to Heart this pattern may be transmitted from Heart-Yang deficiency. In the case of Water overflowing to Lungs this pattern may be transmitted from Lung-Qi deficiency together with Phlegm-Fluids in the Interior. Figures 37.16 and 37.17 summarize the precursors of Kidney-Yang deficiency, Water overflowing.
Typically develops into: This pattern is an extreme pattern, which, itself, usually derives from other patterns.
Easy clinical tip: Oedema of the ankles, breathlessness, a Deep-Weak-Slow pulse and a Pale-Swollen tongue are enough to diagnose Kidney-Yang deficiency, Water overflowing.

Treatment

Principle: tonify and warm the Kidneys, transform Water, warm and tonify Spleen-Yang (and Heart-Yang or Lung-Qi if Water has overflowed to Heart or Lungs)

Du-4 Strengthens the Fire of the Gate of Life, which promotes the transformation of Water.
BL-23 Tonifies Kidney-Yang.
BL-22 Stimulates the transformation of fluids in the Lower Burner.
BL-20 With moxa, tonifies Spleen-Yang.
Ren-9 Promotes the transformation of fluids.
ST-28 Promotes the transformation of fluids in the Lower Burner.
SP-9 With SP-6, resolves Dampness from the Lower Burner.
SP-6 With SP-9, resolves Dampness from the Lower Burner.
KI-7 Tonifies Kidney-Yang and resolves oedema.
Du-14 For Water overflowing to the Heart: with direct moxa, tonifies Heart-Yang.
BL-15 For Water overflowing to the Heart: with moxa, tonifies Heart-Yang.
LU-7 For Water overflowing to the Lungs: stimulates the Lung's function of dominating the Water passages and resolves oedema.
BL-13 For Water overflowing to the Lungs: with Du-12, tonifies Lung-Qi.

Combination Points (and notes)

  • A specific needling technique here goes beyond reinforce/reduce: use thick needles and leave the points open after withdrawal so a few drops of fluid can escape β€” a physical, not just energetic, approach to draining oedema.
  • The point set branches cleanly into a fixed base (tonify Kidney-Yang/Spleen-Yang, reduce to transform Water) plus one add-on module depending on which organ the Water has overflowed to β€” Heart (Du-14, BL-15) or Lungs (LU-7, BL-13, Du-12).

Kidney-Yin deficiency, Empty-Heat blazing

Clinical Presentation

Malar flush, mental restlessness, insomnia, night sweating, low-grade fever, afternoon fever, five-palm heat, feeling of heat in the afternoon and/or evening, scanty dark urine, blood in the urine, dry throat especially at night, thirst with desire to drink in small sips, dizziness, tinnitus, hardness of hearing, lower backache, nocturnal emissions with dreams, excessive sexual desire, dry stools.

Tongue: Red, cracked with a red tip, without coating. Pulse: Floating-Empty and Rapid.
Important symptoms: malar flush, dizziness, tinnitus, feeling of heat in the afternoon, Red tongue without coating.
Arises from: This pattern always derives from Kidney-Yin deficiency.
Typically develops into: This pattern does not have many pathological developments as it is itself an end-result pattern. If Empty-Heat is pronounced, it leads to bleeding.
Easy clinical tip: Even only tinnitus and a Red tongue without coating would be enough to diagnose Kidney-Yin deficiency with Empty-Heat.

Treatment

Principle: nourish Kidney-Yin, clear Empty-Heat, calm the Mind

KI-3 Tonifies the Kidneys.
KI-6 With KI-10, nourishes Kidney-Yin.
KI-10 With KI-6, nourishes Kidney-Yin.
KI-9 Tonifies Kidney-Yin and calms the Mind.
Ren-4 Nourishes Kidney-Yin and calms the Mind.
KI-2 Clears Empty-Heat from the Kidneys.
SP-6 Nourishes Kidney-Yin and calms the Mind.
HE-5 With LU-7, conducts Heat downwards away from the head, where it disturbs the Mind.

  • The Method here is a clean two-group split β€” reinforce the six Yin-nourishing points (KI-3, KI-6, KI-9, KI-10, Ren-4, SP-6), reduce everything else β€” worth teaching as the template for every Yin-deficiency-with-Empty-Heat pattern.
  • 'Positively no moxa' is emphatic phrasing worth remembering β€” a reminder that Empty-Heat patterns are the clearest case where moxa would actively work against the treatment.

Kidney- and Liver-Yin deficiency

Tongue: Normal-coloured without coating, or with rootless coating. Pulse: Floating-Empty.
Important symptoms: dry eyes, dry throat, night sweating, scanty menstruation, tongue without coating.
Arises from: This pattern may develop from Liver-Blood and Liver-Yin deficiency. most common consequence of this pattern is the of Empty-Heat. of
Typically develops into: The flaring of Empty-Heat may heat the Blood and cause bleeding.
Easy clinical tip: Even just dry eyes, dry throat, backache and a tongue without coating are enough to diagnose Kidney- and Liver-Yin deficiency.

Treatment

Principle: nourish Kidney- and Liver-Yin

KI-3 Tonifies the Kidneys (carried over from the Kidney-Yin deficiency point set).
KI-6 Nourishes Kidney-Yin (carried over from the Kidney-Yin deficiency point set).
Ren-4 Nourishes Kidney-Yin (carried over from the Kidney-Yin deficiency point set).
BL-23 Tonifies the Kidneys β€” particularly important in chronic conditions.
KI-13 Nourishes Kidney-Yin.
SP-6 Nourishes Liver- and Kidney-Yin.
BL-10 Used specifically for occipital headache.

  • Reconstruction note: this pattern's own point list was garbled in extraction, so the four carried-over points (KI-3, KI-6, Ren-4, in addition to SP-6) reflect the fact that Kidney-Yin symptoms here are treated with the same points already given under Kidney-Yin deficiency β€” worth double-checking this pattern's full point set against the original text before teaching it as definitive.
  • The headache location is a genuine differentiator taught elsewhere on this deck: occipital headache relates to the Kidney, vertex headache to the Liver channel, and temporal throbbing headache signals that Liver-Yang has actually started to rise on top of the Yin deficiency.

Kidneys and Heart not harmonized

Tongue: Red with redder tip without coating, midline Heart crack. Pulse: Floating-Empty and Rapid, or Deep-Weak on both Rear positions and relatively Overflowing on both Front positions.
Important symptoms: palpitations, dizziness, tinnitus, night sweating, Red tongue with redder tip and midline crack and without coating.
Arises from: This pattern usually develops from Kidney-Yin defi
Typically develops into: The flaring of Empty-Heat may heat the Blood and cause bleeding.
Easy clinical tip: Palpitations, tinnitus, night sweating and a Red tongue with a redder tip, midline crack and no coating are enough to diagnose Kidneys and Heart not harmonized.

Treatment

Principle: nourish Kidney- and Heart-Yin, clear Heart Empty-Heat, calm the Mind

HE-7 Calms the Mind.
HE-5 Conducts Heat downwards away from the head, where it disturbs the Mind.
Yintang Calms the Mind.
BL-15 Clears Heart-Heat.
Ren-15 Calms the Mind and nourishes Heart-Yin.
Du-24 Calms the Mind.
KI-3 With KI-9 and KI-10, tonifies Kidney-Yin.
KI-9 With KI-3 and KI-10, tonifies Kidney-Yin; in particular calms the Mind.
KI-10 With KI-3 and KI-9, tonifies Kidney-Yin.

Combination Points (and notes)

  • HE-5 and BL-15 stand out as the only two points here that reduce rather than reinforce β€” both work by clearing/conducting away Heart-Heat, while every other point nourishes Yin or calms the Mind directly.
  • This is the classic 'Water not controlling Fire' picture β€” Kidney-Yin deficient below fails to keep Heart-Fire in check above, producing the combination of Kidney signs (backache, tinnitus) with Heart signs (palpitations, insomnia, a Heart-crack tongue) together.

Kidney- and Lung-Yin deficiency

Clinical Presentation

Dry cough which is worse in the evening, dry throat and mouth, thin body, breathlessness on exertion, lower backache, night sweating, dizziness, tinnitus, hardness of hearing, scanty urination.

Tongue: Normal-coloured without coating, or with rootless coating. Pulse: Floating-Empty.
Important symptoms: dry cough, dizziness, tinnitus, night sweating, tongue without coating.
Arises from: Obviously this pattern usually derives from Kidney-Yin deficiency; however, it may also develop from a chronic condition of Lung-Yin deficiency.
Typically develops into: This pattern will eventually lead to the development of Empty-Heat and therefore the possibility of bleeding from Blood-Heat.
Easy clinical tip: A dry cough, tinnitus and a tongue without coating would be enough to diagnose Kidney- and Lung-Yin deficiency.

Treatment

Principle: nourish Lung- and Kidney-Yin, nourish Body Fluids

KI-3 Tonifies Kidney-Yin.
KI-6 In combination with LU-7, opens the Directing Vessel, benefits the throat, stimulates the Kidney's reception of Qi and tonifies Lung- and Kidney-Yin.
LU-7 In combination with KI-6, opens the Directing Vessel and tonifies Lung- and Kidney-Yin.
Ren-4 With KI-13, nourishes Kidney-Yin and Essence.
KI-13 With Ren-4, nourishes Kidney-Yin and Essence.
LU-9 Nourishes Lung-Yin.
LU-1 Nourishes Lung-Yin, restores the descending of Lung-Qi and stops cough.
SP-6 Nourishes Kidney-Yin and promotes fluids.
BL-43 Tonifies Lung-Yin β€” specific for chronic conditions.

Arises from: This pattern develops frequently from a chronic condition of Spleen-Yang deficiency.
Typically develops into: When it persists for a long time, this pattern frequently leads to the formation of Dampness and/or Phlegm. Also, if this combined pattern is chronic, it may lead to the pattern of Kidney-Yang deficiency, Water overflowing.
