Anal Fissure

How Chinese medicine differentiates an anal fissure into the six patterns you'll actually meet in practice, and the points for each. Distinct from this site's Hemorrhoids and Constipation pages. Acupuncture only β€” no herbs.

This page is educational, for practitioners and students β€” it is not a diagnostic tool and does not replace a proper in-person assessment. An anal fissure that does not heal within a few weeks, or comes with heavy bleeding, needs medical assessment.
General Acupuncture Points

Acupuncture points traditionally used for an anal fissure β€” 3 key points, used across the patterns below rather than tied to just one.

PointKey action
SP-6Strengthens the Spleen. Resolves Dampness. Promotes the function of the Liver and the smooth flow of Liver-Qi.
ST-36Benefits Stomach and Spleen. Tonifies Qi and Blood. Tonifies the Original Qi.
KI-3Tonifies the Kidneys (both Yin and Yang). Strengthens the Kidney’s receiving of Qi. Calms the Mind.
The Six Patterns

1. Large Intestine Dryness View full pattern →

Clinical manifestations: Dry, hard, pellet-like stools requiring straining; sharp cutting pain; dry mouth and throat; slow healing.
Tongue: Red, thin, dry with central cracks, thin dry slightly yellow coating. Pulse: Fine, choppy, slightly rapid.
Treatment principle: Generates fluids and moistens the intestines to soften stool and prevent re-tearing.
Note: no moxa. Linked to this site's own Large-Intestine-Dry slide.

2. Damp-Heat in the Large Intestine View full pattern →

Clinical manifestations: Sharp burning pain; bright red bleeding; a swollen, hot, damp anal area; loose, sticky, foul-smelling stools; an incomplete-evacuation sensation; body heaviness and a stuffy chest.
Tongue: Red with thick, yellow, greasy coating, especially at the centre and root. Pulse: Slippery and rapid.
Treatment principle: Clears Heat, dries Dampness and stops bleeding to eliminate the inflamed, moist environment.
Note: no moxa. Linked to this site's own Damp-Heat-in-the-Large-Intestine slide.

3. Qi and Blood Stagnation

Clinical manifestations: Fixed, stabbing pain worsening with pressure or straining; dark purple or bluish fissure edges; a chronic, slow-healing tear; irritability and emotional tension.
Tongue: Dark purple, possible stasis spots, distended dark sublingual veins. Pulse: Wiry and choppy.
Treatment principle: Invigorates Blood circulation and dispels stagnation to nourish the wound and relieve pain.
Note: no moxa. No combined Qi-and-Blood-stagnation slide exists on this site's decks β€” no pattern link is given here.

4. Heat in the Blood

Clinical manifestations: Bright red, copious bleeding; burning anal pain; an internal heat sensation worse at night; thirst desiring cold drinks; restlessness and irritability.
Tongue: Red with red spots or prickles, thin yellow dry coating. Pulse: Rapid, wiry, slippery.
Treatment principle: Cools the Blood and stops reckless bleeding to reduce inflammation and promote healing.
Note: no moxa. This site's decks have no standalone Heat-in-the-Blood slide β€” no pattern link is given here.

5. Qi and Blood Deficiency

Clinical manifestations: An extremely slow-healing fissure; fatigue and low energy; a pale complexion, lips and nails; dizziness; poor appetite; shortness of breath on exertion.
Tongue: Pale, possibly thin with teeth marks, thin white coating. Pulse: Fine, weak.
Treatment principle: Simultaneously tonifies Qi and nourishes Blood to provide resources for tissue repair.
Note: moxa is appropriate. No combined Qi-and-Blood-deficiency slide exists on this site's decks β€” no pattern link is given here.

6. Yin Deficiency View full pattern →

Clinical manifestations: A dry, deep fissure; hard, pellet-like stools; a dry mouth and throat worse at night; night sweats; a sensation of heat in the palms, soles and chest; overwork worsens the condition.
Tongue: Red, thin with cracks, little to no coating, dry surface. Pulse: Fine, rapid.
Treatment principle: Nourishes Kidney Yin and generates fluids to moisten tissues and ease constipation-related tearing.
Note: no moxa (Yin deficiency with heat signs). Linked to this site's own Kidney-Yin-deficiency slide as the closest general match.
