Breech Presentation (Fetus Malposition)

How Chinese medicine differentiates fetal malposition, the five patterns you'll actually meet in practice, and the points for each. Moxibustion only — no needling for this indication, and acupuncture only elsewhere on this page; no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Moxibustion for breech presentation is specifically indicated between 32–36 weeks under professional guidance and coordinated with the patient's obstetric provider — never attempted on unstable, high-risk, or multiple pregnancies without medical clearance. SP-6 and L.I.-4 are otherwise generally avoided in pregnancy because they can stimulate uterine activity — that is precisely why they appear here, used deliberately and only in this specific late-pregnancy context.
General Acupuncture Points

Acupuncture points traditionally used for fetal malposition — 6 key points, used across the patterns below rather than tied to just one.

PointKey action
BL-67Subdues rebellious Qi from the head, clears Heat, promotes labour — moxibustion is the classic technique for turning a breech fetus.
BL-23Tonifies the Kidneys and nourishes Kidney-Essence, consolidates Kidney-Qi.
KI-3Tonifies the Kidneys (both Yin and Yang), strengthens the lower back, regulates the uterus.
SP-6Strengthens the Spleen, resolves Dampness, regulates the uterus and menstruation, moves Blood.
ST-36Benefits Stomach and Spleen, tonifies Qi and Blood, raises Yang.
LR-3Promotes the smooth flow of Liver-Qi, invigorates Blood and regulates menstruation.
BL-17Invigorates Blood, nourishes Blood, an Influential point for Blood.
The Five Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Kidney Qi Deficiency

Clinical manifestations: Deep, aching soreness in lower back and knees; extreme fatigue and lack of stamina; frequent urination especially at night with clear urine; shortness of breath with mild exertion; dizziness or heaviness in legs.
Tongue: Pale, slightly puffy body with possible tooth marks and thin white coating. Pulse: Deep, weak, and fine, weakest at the rear positions.
Treatment principle: Tonify Kidney Qi to provide the vital energy needed for fetal rotation.
Note: moxa applicable and preferred throughout. This site's Kidney-Qi not Firm slide is framed around incontinence and prolapse, not this presentation's clear-urine fatigue picture — always read the actual deck content rather than the name, so no pattern link is given here.

2. Spleen Deficiency with Dampness

Clinical manifestations: Heaviness in abdomen and limbs; loose stools or diarrhea; abdominal bloating worse after eating; poor appetite and fatigue; swelling in legs; large-for-dates fundal height.
Tongue: Pale, swollen body with teeth marks, white greasy coating, moist surface. Pulse: Soggy and slow, possibly slippery.
Treatment principle: Strengthen the Spleen's transformative function to resolve dampness and restore lifting Qi.
Note: moxa applicable. No deck on this site combines Spleen deficiency with Dampness in one slide, so no pattern link is given here — the same absence found for Gastroparesis.

3. Liver Qi Stagnation View full pattern →

Clinical manifestations: Distension or bloating along ribs and flanks; irritability, mood swings, or short temper; frequent sighing; breast tenderness or swelling; lower abdominal tension worse with stress.
Tongue: Normal or slightly dark body with red or slightly purple sides, thin white coat. Pulse: Wiry, especially tense at left Guan.
Treatment principle: Smooth Liver Qi flow to relax the uterine environment.
Note: no moxa except on BL-67. Direct match on the core distension/irritability/sighing picture.

4. Qi and Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing or heavy sensation in lower abdomen; emotional tension, irritability, or feeling stuck; baby's movements feel restricted or cause discomfort.
Tongue: Dark purple body with stasis spots and distended sublingual veins, thin white coat. Pulse: Wiry and choppy.
Treatment principle: Invigorate Qi and Blood circulation to dissolve the physical barrier of stagnation.
Note: no moxa except on BL-67. Matches Liver-Blood stasis — a dark purple tongue with stasis spots is the same discoloration marker named there.

5. Blood Deficiency and Stagnation

Clinical manifestations: Fixed dull ache in lower abdomen; history of scanty, dark-clotted periods; dizziness and heart palpitations; fatigue and low energy.
Tongue: Pale body with purple or dusky cast, thin, possible stasis spots, sublingual veins distended. Pulse: Fine and choppy, often wiry on left Guan.
Treatment principle: Nourish blood while invigorating circulation.
Note: moxa applicable. No deck on this site combines Blood deficiency with stagnation in one slide, so no pattern link is given here.
