Buttocks Pain

How Chinese medicine differentiates buttocks pain — distinct from Sciatica and Piriformis Syndrome already covered — the five patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Buttocks pain with loss of bladder or bowel control needs urgent medical evaluation before assuming it is a simple pattern.
General Acupuncture Points

Acupuncture points traditionally used for buttocks pain — 9 key points, used across the patterns below rather than tied to just one.

PointKey action
G.B.-30Removes obstructions from the channel, resolves Dampness and expels Wind.
BL-40Clears Heat and cools Blood, removes obstructions from the channel.

BL-23Tonifies Kidney-Yang and nourishes Kidney-Yin.
SP-10Cools the Blood, invigorates Blood and removes stagnation.
LR-3Subdues Liver-Yang, clears interior Wind.
G.B.-34Resolves Liver-Qi stagnation, resolves Damp-Heat in Liver and Gall Bladder.
DU-4Tonifies Kidney-Yang, warms the Gate of Life, expels Cold.
REN-4Warms the lower burner, fortifies foundational Yang.
BL-32Local point, relieves pain and stagnation in the buttocks and sacral region.

The Five Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Painful Obstruction with Wind-Cold-Damp

Clinical manifestations: Deep aching pain worsening with cold or wind, stiffness and limited hip range of motion, heaviness or numbness in the buttock, pain relieved by warmth and gentle movement, aversion to cold or drafts.
Tongue: Pale, slightly swollen body with teeth marks and white greasy coating. Pulse: Wiry and tight, or slow and soggy.
Treatment principle: Warm the Spleen and Kidney Yang, dispel Cold, and dry Dampness to unblock the channels in the buttocks.
Note: moxa applicable on the tonifying points given the Cold. No deck on this site has a combined Wind-Cold-Damp Bi-syndrome slide, a confirmed and recurring absence, so no pattern link is given here.

2. Qi and Blood Stagnation View full pattern →

Clinical manifestations: Fixed stabbing pain in the buttock, pain radiates down the thigh or calf, worse with prolonged sitting or inactivity, worse with pressure or touch, irritability or frustration, history of injury or poor posture.
Tongue: Purple or dark body with stasis spots, distended sublingual veins, thin white coat. Pulse: Wiry and choppy.
Treatment principle: Move Qi and Blood in the channels, especially for fixed stabbing pain in the buttocks and legs.
Note: no moxa. Matches this site's Liver-Blood stasis slide — a purple/dark tongue with stasis spots is the same discoloration marker named there.

3. Painful Obstruction due to Damp Heat in Channels

Clinical manifestations: Buttock feels hot and swollen, pain worsens with warmth, heavy aching sensation, thirst and dark urine, irritability.
Tongue: Red body, possibly swollen, with yellow greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clear Damp-Heat from the lower body channels and relieve pain.
Note: no moxa. No deck on this site has a dedicated Damp-Heat-in-channels Bi-syndrome category, a confirmed and recurring absence, so no pattern link is given here.

4. Kidney-Yang Deficiency View full pattern →

Clinical manifestations: Cold dull aching pain in the buttocks, pain better with warmth and worse with cold, frequent urination especially at night, low energy and desire to sleep, soreness and weakness in the lower back and knees.
Tongue: Pale, puffy, tender body with teeth marks and wet white coating. Pulse: Deep, slow and weak, especially at rear positions.
Treatment principle: Tonify Kidney-Yang to strengthen the body's ability to warm and protect the lower back.
Note: moxibustion strongly indicated throughout. Direct match with this site's own Kidney-Yang-deficiency slide.

5. Qi and Blood Deficiency

Clinical manifestations: Dull persistent ache, worse after exertion or prolonged standing, general fatigue and weakness, pale complexion and lips, dizziness or lightheadedness, numbness or tingling in the limbs.
Tongue: Pale body, may be thin or have teeth marks, thin white coating. Pulse: Fine and weak, especially at right Guan.
Treatment principle: Tonify both Qi and Blood, rebuilding the nourishment that the buttock muscles and sinews need.
Note: moxa applicable on the tonifying points. No deck on this site combines Qi and Blood deficiency into one slide, so no pattern link is given here.
