Clinical manifestations: Pain that moves or fluctuates with emotional state; a tight, distending sensation around the ribcage; frequent sighing or a feeling of a lump in the throat; irritability, mood swings, or feeling "on edge"; pain that is worse with stress and better with relaxation; a worsening after prolonged inactivity; brought on by stress, frustration, or suppressed emotion; eased by gentle exercise, emotional release, or a calmer routine.
Tongue: Normal or slightly dark, red or slightly purple sides, thin white coating. Pulse: Wiry, like a taut guitar string.
Treatment principle: Course the Liver, resolve constraint, move Qi and alleviate pain, invigorate Blood and dispel stasis.

3. Painful Obstruction with Qi and Blood Deficiency
Clinical manifestations: Dull, aching pain in the muscles and joints; numbness or tingling in the limbs; fatigue and a lack of strength; a pale face and lips; palpitations and shortness of breath; pain that worsens after exertion; brought on by chronic illness, long-term overwork, or an underlying constitutional weakness; eased by rest and gentle activity.
Tongue: Pale, possibly thin with tooth marks, thin white coating. Pulse: Fine, Weak, often Slow, weaker at the rear positions.
Treatment principle: Tonify Qi and warm the channels, harmonize the Nutritive and Defensive Qi, nourish Blood and dispel obstruction.

4. Painful Obstruction with Liver and Kidney Deficiency
Clinical manifestations: A dull, chronic ache or pain in the muscles and joints; soreness and weakness in the lower back and knees; numbness or tingling in the limbs; a preference for warmth and an aversion to cold; fatigue, poor stamina, and shortness of breath on exertion; a pale or dull complexion; pain that worsens in cold, damp, or rainy weather, or with overexertion; brought on by ageing, chronic illness, or long-standing Liver and Kidney weakness; eased by warmth and rest.
Tongue: Pale, thin, thin white coating. Pulse: Fine, Weak, Slow, especially weak at the Chi (rear) position.
Treatment principle: Dispel Wind-Dampness, relieve Painful Obstruction, supplement the Liver and Kidneys.

5. Phlegm in the Channels, Joints and Muscles
Clinical manifestations: Fixed, stabbing pain in the muscles; firm or hard swellings near the joints; purple or dark skin around the painful areas; subcutaneous nodules or lumps; numbness or a heavy sensation in the limbs; pain that worsens in cold, damp, or rainy weather, or with prolonged inactivity; brought on by long-standing untreated Blood stasis combined with chronic Phlegm accumulation; eased by warmth and gentle movement.
Tongue: Purple with stasis spots, white greasy coating, distended sublingual veins. Pulse: Wiry and Choppy, or Wiry and Slippery if Phlegm predominates.
Treatment principle: Scour Phlegm and open the orifices, clear Heat and transform Phlegm, regulate Qi and resolve turbidity.

Tongue: Red, possibly slightly swollen, yellow greasy coating. Pulse: Slippery and Rapid, possibly Soggy or Wiry at the Guan position.
Treatment principle: Clear Heat and drain Dampness, unblock the channels, relieve Painful Obstruction.
