Joint Stiffness

How Chinese medicine differentiates general joint stiffness into the six patterns you'll actually meet in practice, and the points for each. Distinct from this site's own Osteoarthritis and Ankylosing Spondylitis pages. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. New joint swelling, redness, or stiffness lasting over an hour each morning deserves a rheumatology work-up rather than assuming a simple pattern.
General Acupuncture Points

Acupuncture points traditionally used for joint stiffness — 10 key points, used across the patterns below rather than tied to just one.

PointKey action
KI-3Tonifies the Kidneys (both Yin and Yang). Calms the Mind. Benefits Essence. Strengthens the lower back and knees.
SP-6Strengthens the Spleen. Nourishes Blood and Yin.
ST-36Benefits Stomach and Spleen. Tonifies Qi and Blood. Expels Wind and Damp.
BL-20Tonifies Spleen and Stomach. Resolves Dampness. Nourishes Blood.
SP-10Cools Blood. Invigorates Blood and eliminates stasis.
LR-3Subdues Liver-Yang. Promotes the smooth flow of Liver-Qi. Invigorates Blood.
L.I.-4Removes obstructions from the channel. Stops pain. Regulates the Defensive Qi.
L.I.-11Clears Heat and cools Blood. Removes obstructions and Heat from the channel. Benefits the sinews and joints.
SP-9Regulates the Spleen. Resolves Dampness. Opens the Water passages.
ST-40Resolves Phlegm and Dampness. Opens the chest and subdues rebellious Qi.
The Six Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Painful Obstruction with Wind-Cold-Damp

Clinical manifestations: Joint pain and stiffness worse in cold, damp, or rainy weather, heaviness or numbness in the limbs, pain relieved by warmth and gentle movement, swelling without redness or heat, aversion to cold or wind.
Tongue: Pale, slightly swollen with teeth marks, white greasy coating, moist. Pulse: Wiry and tight, or soggy and slow if Dampness predominates.
Treatment principle: Warms the channels, disperses Cold, eliminates Dampness to restore joint mobility.
Note: moxa strongly indicated. This is the classical Wind-Cold-Damp Bi (painful obstruction) syndrome, with no corresponding channel/joint-invasion slide on any deck here, a gap confirmed absent many times over this session — no pattern link is given here; points still link fine individually, alongside local points at the affected joint.

2. Painful Obstruction Due to Damp-Heat in the Channels

Clinical manifestations: Joints are red, swollen, and hot to the touch, heavy aching sensation in the limbs, thirst, irritability, dark scanty urine, stiffness worse with warmth and better with cold.
Tongue: Red body, possibly swollen, with yellow greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clears Heat, drains Dampness, unblocks the channels and collaterals.
Note: no moxa. This is the Damp-Heat variant of the same Bi-syndrome gap as pattern 1 above — no dedicated channel/joint Damp-Heat slide exists on any deck — no pattern link is given here.

3. Painful Obstruction with Liver and Kidney Deficiency View full pattern →

Clinical manifestations: Dull aching joint pain, lower back and knee weakness, morning stiffness easing with gentle movement, worse in cold and damp weather, fatigue, numbness or tingling in the limbs, pale complexion.
Tongue: Pale, thin body with thin white coating. Pulse: Fine, weak, and slow, especially weak at the Kidney position.
Treatment principle: Tonifies Liver and Kidney, strengthens sinews and bones, dispels Wind-Dampness.
Note: moxa applicable. This site has no Bi-syndrome-specific Liver-and-Kidney-deficiency slide; linked to the Kidney-and-Liver-Yin-deficiency slide as the closest deficiency-mechanism match, flagged since that slide's own text doesn't frame it as a joint/Bi pattern.

4. Painful Obstruction with Qi and Blood Deficiency View full pattern →

Clinical manifestations: Dull aching joint pain worse after activity, numbness or tingling in the limbs, pale complexion, fatigue and weakness, poor appetite, spontaneous sweating.
Tongue: Pale body, possibly thin with teeth marks, thin white coating. Pulse: Fine and weak, possibly slow.
Treatment principle: Tonifies Qi and Blood, warms the channels, nourishes sinews and joints.
Note: moxa applicable. This site's decks have no combined Qi-and-Blood-deficiency slide, a gap confirmed absent many times over this session; linked to the Spleen-Qi-deficiency slide for the Qi component, flagged for the Blood component not being isolated there.

5. Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing joint pain that does not move, pain and stiffness worsen at night or after rest, worse with pressure on the joint, dark purplish tongue with stasis spots, history of old injury or chronic pain.
Tongue: Dark purplish body with stasis spots, distended sublingual veins. Pulse: Choppy.
Treatment principle: Invigorates Blood, dispels stasis, unblocks the channels and alleviates pain.
Note: no moxa. Linked to this site's own Liver-Blood-stasis slide per this session's discoloration-marker rule.

6. Phlegm in the Channels, Joints and Muscles

Clinical manifestations: Fixed stabbing pain in the joints, hard joint swelling, subcutaneous nodules or lumps near joints, numbness or heavy sensation in the limbs, chest tightness or excessive phlegm.
Tongue: Purple body with stasis spots, white greasy coating, distended sublingual veins. Pulse: Wiry and choppy, or wiry and slippery if Phlegm predominates.
Treatment principle: Transforms Phlegm, invigorates Blood, unblocks the collaterals to relieve stubborn stiffness.
Note: no moxa. This site's decks have no dedicated Phlegm-in-the-channels/joints/muscles slide, a gap confirmed absent many times over this session — no pattern link is given here.
