Lower Abdominal Pain

How Chinese medicine differentiates lower abdominal pain into the six patterns you'll actually meet in practice, and the points for each. Distinct from this site's Abdominal Pain and Chronic Pelvic Pain pages. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Sudden severe lower abdominal pain, pain with fever, vomiting, vaginal bleeding or a possible pregnancy needs urgent medical assessment.
General Acupuncture Points

Acupuncture points traditionally used for lower abdominal pain — 3 key points, used across the patterns below rather than tied to just one.

PointKey action
LIV-3Subdues Liver-Yang. Extinguishes interior Wind. Promotes the smooth flow of Liver-Qi.
SP-6Strengthens the Spleen. Resolves Dampness. Promotes the function of the Liver and the smooth flow of Liver-Qi.
REN-4Nourishes Blood and Yin. Strengthens the Kidneys. Strengthens the Uterus and regulates menstruation.
The Six Patterns

1. Liver Qi Stagnation View full pattern →

Clinical manifestations: Distending, wandering pain that comes and goes; worse with stress; frequent sighing or belching; irritability or mood swings; breast tenderness; irregular or painful periods.
Tongue: Normal or slightly dark, red or purplish sides, thin white coat. Pulse: Wiry.
Treatment principle: Smooths Liver Qi, relieves distension and harmonizes Liver and Spleen.
Note: no moxa.

2. Stagnation of Cold in the Liver Channel View full pattern →

Clinical manifestations: Cold, cramping pain that improves with warmth; radiates to the groin, testicles or inner thigh; worse with cold or iced drinks; cold hands and feet; scrotal contraction.
Tongue: Pale, wet white coat. Pulse: Deep, wiry, slow or tight.
Treatment principle: Warms the Liver channel, expels Cold and stops pain.
Note: moxa is strongly indicated.

3. Damp-Heat in the Lower Burner View full pattern →

Clinical manifestations: Burning or hot sensation in the lower abdomen; pain worse with pressure; dark, scanty or painful urination; foul vaginal discharge or scrotal dampness; sticky stools; heavy, dragging feeling.
Tongue: Red, slightly swollen, thick yellow greasy coat, thickest at the root. Pulse: Slippery and rapid, more pronounced at the rear position.
Treatment principle: Drains Damp-Heat from the Lower Burner and stops burning pain.
Note: no moxa. Linked to this site's own Damp-Heat-in-the-Bladder slide, its established Lower-Burner Damp-Heat convention.

4. Spleen Yang Deficiency View full pattern →

Clinical manifestations: Dull, lingering pain improved by warmth and pressure; loose stools with undigested food; cold hands and feet; fatigue and heaviness.
Tongue: Pale, puffy, tender, tooth-marked, white slippery coat. Pulse: Deep, slow and weak.
Treatment principle: Warms the Middle Burner, disperses Cold and restores Spleen Yang.
Note: moxa is strongly indicated.

5. Blood Accumulation in the Lower Burner

Clinical manifestations: Fixed stabbing pain in the lower abdomen; tenderness and hardness on pressure; restlessness, confusion or manic agitation; normal urination.
Tongue: Purple or dark with stasis spots, dry yellow coat, engorged sublingual veins. Pulse: Deep and choppy.
Treatment principle: Breaks up Blood stasis in the Lower Burner, clears Heat and relieves pain and mental disturbance.
Note: Excess. Needs medical exclusion of acute surgical causes first. No matching deck slide (a Blood-stasis-with-Heat-in-the-Uterus pattern is not among the decks) — no pattern link is given here.

6. Blood Deficiency and Stagnation

Clinical manifestations: Dull or stabbing lower abdominal pain, better with gentle pressure; scanty, late periods with dark clots; pale or sallow face with a dusky tinge; dizziness, fatigue, palpitations; brittle or ridged nails.
Tongue: Pale with a purple cast, thin, possible stasis spots, thin white coat. Pulse: Fine and choppy, often wiry at the left middle position.
Treatment principle: Nourishes Blood and invigorates its flow, addressing both the deficiency and the stasis.
Note: Mixed Deficiency/Excess. No deck slide combines Blood deficiency with stagnation — no pattern link is given here.
