Nocturia

How Chinese medicine differentiates waking at night to urinate — distinct from Overactive Bladder and Frequent Urination already covered — the six patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Nocturia with pain, fever, or blood in the urine needs medical evaluation before assuming it is a simple pattern.
General Acupuncture Points

Acupuncture points traditionally used for nocturia — 9 key points, used across the patterns below rather than tied to just one.

PointKey action

DU-4Moxa ideal here to stoke Ming Men fire.
REN-4Fortifies Kidney-Yang and consolidates the Bladder gate.
REN-6Raises Qi and supports the Kidneys' ability to hold urine.
BL-20Reinforces Spleen-Yang to improve fluid transformation.
ST-36Strengthens Spleen and Stomach to boost overall Yang energy.
REN-3Bladder's front-Mu point, regulates the Bladder, stops leakage when warmed.
SP-6Regulates water passages and supports Spleen and Kidney in managing fluids.
SP-9Drains Dampness and promotes urination.
BL-28Back-Shu point of the Bladder, clears Damp-Heat and restores normal function.
KI-3Source point, strongly nourishes Kidney-Yin and anchors Essence.
KI-6Nourishes Yin, benefits the throat, regulates nighttime urination.
The Six Patterns

1. Kidney-Yang Deficiency View full pattern →

Clinical manifestations: Clear, watery, copious urine at night, deep aching cold in the lower back and knees, feeling cold all over especially the lower body, fatigue and low spirits.
Tongue: Pale and puffy with a wet white coating, teeth marks on the edges. Pulse: Deep, slow and weak, especially at the rear position.
Treatment principle: Warm Kidney-Yang and restore the Bladder's ability to hold urine through the night.
Note: moxa strongly indicated throughout. Direct match with this site's own Kidney-Yang-deficiency slide.

2. Kidney-Qi Not Firm View full pattern →

Clinical manifestations: Clear dilute urine, dribbling after urination, sore and weak lower back, fatigue and lack of strength, nocturia without marked cold limbs.
Tongue: Pale, puffy, moist body with thin white coat and possible tooth marks. Pulse: Deep, weak and fine, especially feeble at both Chi positions.
Treatment principle: Tonify Kidney-Qi and consolidate the Bladder gate to prevent leakage.
Note: moxibustion especially helpful on BL-23. Direct match with this site's own Kidney-Qi-not-Firm slide — its own symptom list names dribbling after urination and nighttime urination exactly.

3. Kidney and Spleen Yang Deficiency View full pattern →

Clinical manifestations: Frequent urination with clear pale urine, chronic loose stools or early-morning diarrhea, abdominal bloating after eating, cold lower back and knees, cold hands and feet with aversion to cold.
Tongue: Pale, swollen, puffy body with teeth marks and a white slippery coating. Pulse: Deep, weak and slow, both Chi positions feeble, right Guan weak.
Treatment principle: Warm Spleen-Yang and dispel internal Cold, addressing the digestive root of the fluid imbalance.
Note: moxa strongly indicated throughout. Direct match with this site's own Kidney-and-Spleen-Yang-deficiency slide.

4. Bladder Deficient and Cold View full pattern →

Clinical manifestations: Frequent pale clear urination at night, feeling of cold in the lower abdomen and back, urinary incontinence or bedwetting, nocturia worse in cold weather, dull aching lower back pain.
Tongue: Pale, puffy and wet with a thin white slippery coating. Pulse: Deep, weak and slow, especially feeble at the rear positions.
Treatment principle: Warm the Bladder, consolidate Kidney-Qi, reduce frequent pale urination through direct warming.
Note: moxa especially effective, particularly on BL-23. Direct match with this site's own Bladder-Deficient-and-Cold slide.

5. Damp-Heat in the Bladder View full pattern →

Clinical manifestations: Burning or stinging sensation during urination, dark yellow cloudy or turbid urine, sudden urgent need to urinate even at night, fullness or discomfort in the lower abdomen, thirst with little desire to actually drink.
Tongue: Red body with a yellow greasy coating thickest at the root. Pulse: Slippery and rapid.
Treatment principle: Clear Damp-Heat from the Bladder, relieve burning and urgency, restore normal urination patterns.
Note: no moxa. Direct match with this site's own Damp-Heat-in-the-Bladder slide.

6. Kidney-Yin Deficiency View full pattern →

Clinical manifestations: Scanty dark yellow urine, dry mouth and throat especially at night, night sweats, heat in palms soles and chest, soreness and weakness of the lower back and knees.
Tongue: Red, thin body with little or no coating, dry surface, possible cracks. Pulse: Fine and rapid.
Treatment principle: Nourish Kidney-Yin and clear deficiency Heat, restoring the Kidney's ability to hold fluids at night.
Note: no moxa given the Empty-Heat. Direct match with this site's own Kidney-Yin-deficiency slide.
