Nocturnal Enuresis (Bedwetting)

How Chinese medicine differentiates nocturnal enuresis, the four patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Bedwetting that starts suddenly after a long dry period, or is accompanied by pain or excessive thirst, needs medical evaluation before acupuncture is considered.

The Four Patterns

1. Kidney-Qi Not Firm View full pattern →

Clinical manifestations: Deep-sleep bedwetting without waking; clear and copious urine; cold hands and feet; a sore, weak lower back and knees; fatigue and lack of strength.
Tongue: Pale, puffy, moist, thin white coating, possible tooth marks. Pulse: Deep, weak, Fine.
Treatment principle: Warm Kidney-Yang and astringe urine to firm the Bladder gate.
Note: moxa applicable. Direct match with Kidney-Qi not Firm.

2. Spleen and Lung Qi Deficiency View full pattern →

Clinical manifestations: Bedwetting with daytime dribbling; fatigue and weakness; a poor appetite and loose stools; a pale face and weak voice; shortness of breath with spontaneous sweating.
Tongue: Pale, puffy, tender, teeth marks, thin white slippery coating. Pulse: Weak.
Treatment principle: Tonify Spleen and Lung Qi to restore water metabolism and Bladder control.
Note: moxa applicable. Direct match with Spleen- and Lung-Qi deficiency.

3. Damp-Heat in the Liver Channel View full pattern →

Clinical manifestations: Dark yellow, strong-smelling urine; a bitter taste in the mouth; irritability and restless sleep; genital discomfort or itching.
Tongue: Red with a yellow greasy coating, thicker at the centre and root. Pulse: Wiry, Slippery, Rapid.
Treatment principle: Clear Damp-Heat from the Liver channel to stop the Bladder disturbance.
Note: no moxa. Matches Damp-Heat in the Liver — genital itching and urinary difficulty are named symptoms there too.

4. Disharmony Between Heart and Kidneys View full pattern →

Clinical manifestations: Bedwetting during dreams; difficulty falling asleep or frequent waking; mental restlessness and irritability; vivid or disturbing dreams; heat in the palms, soles and chest; night sweats; a dry mouth and throat at night.
Tongue: Red, thin, redder tip, little or no coating, possible midline crack. Pulse: Fine, Rapid.
Treatment principle: Nourish Heart and Kidney Yin to restore their communication and calm the Mind.
Note: no moxa. Near-exact match with Kidneys and Heart not harmonized — palpitations, dream-disturbed sleep, night sweating and a Red tongue with redder tip and midline crack are shared almost point for point.
