Purple Tongue

How Chinese medicine differentiates a purple tongue — a classic Blood-stasis sign checked on exam — into the five patterns you'll actually meet in practice, and the points for each. Distinct from this site's own Pale Tongue and Cracked Tongue pages. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. A purple tongue is one diagnostic sign among many, and has been studied as a marker of cardiovascular disease — always weigh it alongside the pulse, symptoms, and history.
General Acupuncture Points

Acupuncture points traditionally used when a purple tongue is found — 7 key points, used across the patterns below rather than tied to just one.

PointKey action
SP-10Cools Blood. Invigorates Blood and eliminates stasis.
BL-17Invigorates Blood. Cools Blood. Nourishes Blood.
LR-3Subdues Liver-Yang. Promotes the smooth flow of Liver-Qi. Invigorates Blood.
SP-6Strengthens the Spleen. Nourishes Blood and Yin.
L.I.-4Removes obstructions from the channel. Stops pain.
L.I.-11Clears Heat and cools Blood. Resolves Dampness.
DU-14Clears Heat. Releases the Exterior and expels exterior Wind.
The Five Patterns

1. Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing pain worsening at night, dark menstrual blood with clots, dark purplish lips or nails, rough, dry, scaly skin, dark circles under the eyes.
Tongue: Dusky purple body with possible stasis spots, distended dark sublingual veins, thin white coat. Pulse: Choppy or wiry.
Treatment principle: Invigorate Blood and dispel stasis, move Qi and alleviate pain, open the chest and disperse stagnation.
Note: no moxa. Direct match with this site's own Liver-Blood-stasis slide via the established discoloration-marker rule.

2. Qi and Blood Stagnation View full pattern →

Clinical manifestations: Distending sensation in the chest or rib area, irritability and mood swings, fixed stabbing pain worsening with pressure, dark menstrual blood with clots.
Tongue: Dark purple body with stasis spots and distended sublingual veins, thin white coat. Pulse: Wiry and choppy.
Treatment principle: Move Liver-Qi and Blood, address root stagnation from emotional stress.
Note: no moxa. This is a 2nd, still-within-cap use of this site's own Liver-Blood-stasis slide on this page via the established discoloration-marker rule.

3. Blood Stagnation With Heat

Clinical manifestations: Fixed stabbing pain worsening at night, sensation of internal Heat especially at night, thirst with desire to rinse the mouth but not swallow, dark or clotted menstrual blood, dry, rough, or scaly skin, constipation with dry stools.
Tongue: Reddish-purple body with stasis spots, distended sublingual veins, and a thin dry yellow coating. Pulse: Choppy and rapid, often with a wiry quality.
Treatment principle: Drain Heat and break Blood stasis, cool the Blood and invigorate circulation.
Note: no moxa. Rather than a 3rd use of the Liver-Blood-stasis slide (over this page's two-use cap), this pattern is left without a slide link as redundant with patterns 1 and 2 above; points still link fine individually.

4. Toxic-Heat

Clinical manifestations: Deep purple or purple-black tongue with dry yellow coating, high fever with a burning sensation throughout the body, red, swollen, painful skin boils, abscesses or carbuncles, intense thirst craving cold drinks, restlessness, irritability or mental agitation.
Tongue: Deep purple or purple-black body with prickly thorns and a dry yellow coating. Pulse: Rapid and forceful, often full or overflowing.
Treatment principle: Clear Heat, cool the Blood, and resolve toxin at the Nutritive level.
Note: no moxa; high fever with a deep purple-black tongue needs urgent medical evaluation regardless. This site has no standalone Toxic-Heat slide on any deck — no pattern link is given here; points still link fine individually.

5. Blood Stagnation in the Directing and Penetrating Vessels

Clinical manifestations: Dark purple menstrual blood with clots, fixed stabbing lower abdominal pain worsening with pressure, pain improves after passing clots, engorged, tortuous veins under the tongue, difficulty conceiving or recurrent miscarriage.
Tongue: Dark purple or dusky body, often with purplish spots along the edges, sublingual veins distended, dark, and engorged, thin white coating. Pulse: Choppy, often wiry at the left middle position, may be deep at both rear positions.
Treatment principle: Warm the uterus and dispel Cold, invigorate Blood and dispel stasis, move Qi and alleviate pain in the lower abdomen.
Note: moxa applicable given the Cold-uterus framing. Also left without a slide link as redundant with patterns 1 and 2 above (this page's two-use cap on the Liver-Blood-stasis slide), with the same Chong/Ren-vessel naming nuance flagged as on this site's other postpartum/gynecology pages; points still link fine individually.
