Swollen Tongue

How Chinese medicine differentiates a swollen tongue — distinct from Tongue Pain already covered — the six patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Sudden tongue swelling with difficulty breathing or swallowing is a medical emergency and needs immediate evaluation before assuming it is a simple pattern.
General Acupuncture Points

Acupuncture points traditionally used for a swollen tongue — 8 key points, used across the patterns below rather than tied to just one.

The Six Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Spleen Deficiency with Dampness

Clinical manifestations: Pale puffy tongue with teeth marks, loose stools, abdominal bloating worsening after meals, heaviness in the limbs and body, poor appetite.
Tongue: Pale, swollen, puffy body with teeth marks, white greasy coating, moist surface. Pulse: Soggy and slow, or slippery.
Treatment principle: Tonify Spleen-Qi, strengthen digestive function, and drain accumulated Dampness.
Note: moxa applicable on the tonifying points. No deck on this site combines Spleen-Qi deficiency with Dampness into one slide, so no pattern link is given here.

2. Kidney-Yang Deficiency View full pattern →

Clinical manifestations: Pronounced coldness especially in the lower back and legs, frequent urination with nighttime waking, soreness and cold pain in the lower back and knees, fatigue and low mood.
Tongue: Pale, puffy, tender body with teeth marks and a wet white coating. Pulse: Deep, slow and weak, particularly at the rear position.
Treatment principle: Warm Kidney-Yang, kindle the Gate of Life Fire, and transform retained fluids through warming.
Note: moxibustion recommended on the lower back and abdomen. Direct match with this site's own Kidney-Yang-deficiency slide.

3. Damp-Heat

Clinical manifestations: Red swollen tongue, heaviness in the body and head, sticky greasy mouth sensation, bitter taste, dark scanty urine, loose sticky stools feeling incomplete.
Tongue: Red swollen body with yellow greasy coating difficult to scrape, possible tooth marks. Pulse: Soggy-rapid or slippery-rapid.
Treatment principle: Clear Heat and drain Dampness, transform turbidity, resolve toxicity from the combined Heat-moisture imbalance.
Note: no moxa. This condition names only a generic Damp-Heat pattern rather than a specific organ, and no deck on this site has an organ-nonspecific Damp-Heat slide, so no pattern link is given here.

4. Damp-Phlegm

Clinical manifestations: Thick white greasy tongue coating like paste, pale swollen tongue with scalloped tooth marks, copious white phlegm easily coughed up, body and limb heaviness, chest and upper abdominal stuffiness, brain fog.
Tongue: Pale swollen body with scalloped tooth marks, thick white greasy coating, wet surface. Pulse: Slippery, like a bead rolling through oil.
Treatment principle: Dry Dampness and transform Phlegm, regulate Qi, harmonize the middle burner, direct rebellious Qi downward.
Note: no moxa. No deck on this site has a combined Damp-Phlegm category outside the Lungs, a confirmed and recurring absence, so no pattern link is given here.

5. Phlegm-Fluids

Clinical manifestations: Pale swollen tongue with tooth marks, white greasy slippery coating appearing wet, chest fullness or oppression, gurgling abdominal sounds, dizziness or foggy head, copious clear or white phlegm.
Tongue: Pale swollen body with tooth marks, white greasy slippery coating, excessively moist appearance. Pulse: Wiry, slippery, often deep.
Treatment principle: Warm Yang and transform fluid retention, strengthen the Spleen and resolve Dampness, subdue rushing Qi.
Note: moxa applicable given the underlying Yang deficiency. No deck on this site has a dedicated Phlegm-Fluids slide distinct from Damp-Phlegm, so no pattern link is given here.

6. Blood Stagnation View full pattern →

Clinical manifestations: Dark purple or dusky swollen tongue with stasis spots, fixed stabbing pain anywhere worse at night, dark menstrual blood with clots, dark lips and nails.
Tongue: Dark purple or dusky swollen body with stasis spots, engorged sublingual veins, thin white coat. Pulse: Choppy or wiry-tight.
Treatment principle: Invigorate Blood and dispel stasis, move Qi and alleviate pain, open the chest and disperse stagnation.
Note: no moxa. Matches this site's Liver-Blood stasis slide — a dark purple/dusky tongue with stasis spots is the same discoloration marker named there.
