Apathy

How Chinese medicine differentiates apathy — emotional flatness and loss of motivation — the five patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Persistent apathy can be a sign of depression, dementia, thyroid disease, or other medical conditions and needs proper evaluation before assuming a simple pattern.

The Five Patterns

1. Heart and Spleen Deficiency View full pattern →

Clinical manifestations: Emotional flatness, lack of motivation, poor appetite with bloating, palpitations, disturbed sleep, physical exhaustion, dizziness, sallow complexion.
Tongue: Pale, tender body with teeth marks, thin white coating. Pulse: Fine and weak, possibly choppy.
Treatment principle: Tonify Qi and Blood, strengthen the Spleen, nourish the Heart, calm the Shen.
Note: moxa applicable throughout. Linked to this site's Spleen-and-Heart-Blood-deficiency slide — its own palpitations/poor-memory symptom set lines up closely, the same match already used for Delayed Speech Development and Hallucinations.

2. Liver-Qi Stagnation View full pattern →

Clinical manifestations: Frequent sighing, rib-side distension, irritability alternating with emotional flatness, chest tightness.
Tongue: Normal or slightly dark body with red/purplish sides, thin white coat. Pulse: Wiry, like a taut guitar string.
Treatment principle: Course the Liver, resolve constraint, restore smooth emotional flow, move Qi.
Note: no moxa. Direct exact-name match with this site's own Liver-Qi-stagnation slide.

3. Kidney-Essence Deficiency View full pattern →

Clinical manifestations: Poor memory, mental dullness, lower back and knee weakness, premature greying, tinnitus, night urination, deep fatigue unrelieved by rest.
Tongue: Pale, thin body with little or no coating, possible fine cracks. Pulse: Fine, deep, weak at the rear positions.
Treatment principle: Nourish Kidney-Essence, fill Marrow, replenish Brain nourishment, restore mental vitality.
Note: moxa applicable throughout. Direct exact-name match with this site's own Kidney-Essence-deficiency slide.

4. Obstruction of Spleen by Dampness With Liver-Qi Stagnation View full pattern →

Clinical manifestations: Heavy, foggy-headed feeling, bloating after eating, sticky or incomplete stools, heaviness in the limbs, low mood with frustration, rib-side distension, poor appetite.
Tongue: Pale, swollen body with teeth marks, white greasy excessively moist coating. Pulse: Slippery and wiry.
Treatment principle: Dry Dampness, strengthen the Spleen, move Qi, course the Liver, clear mental fog.
Note: moxa applicable on ST-36 only. Direct exact-name match with this site's own Obstruction-of-Spleen-by-Dampness-with-stagnation-of-Liver-Qi slide — a rare full-title match, previously used for Loss of Taste.

5. Phlegm Misting the Heart

Clinical manifestations: Mental dullness, clouded thinking, heavy-headedness, dizziness, throat gurgling, chest stuffiness, emotional withdrawal.
Tongue: Pale, swollen body with thick white greasy coating, possibly stiff. Pulse: Slippery, like beads on a string.
Treatment principle: Scour Phlegm, open the orifices, transform turbidity, clear consciousness, restore emotional responsiveness.
Note: this chronic foggy-thinking presentation (no unconsciousness, slurred speech or aphasia) is declined against this site's Phlegm-misting-the-Mind slide for the same severity-mismatch reason as OCD and Alzheimer's Disease's identically-named patterns — that slide is framed around acute/stroke-level presentations. No pattern link is given here; points still link fine individually.
