Breast Engorgement

How Chinese medicine differentiates breast engorgement during lactation, the six patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs; frequent, effective milk removal remains the primary treatment.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Fever, a hot/red wedge-shaped area, or a lump that doesn't soften with feeding/pumping needs medical evaluation to rule out mastitis or an abscess (see the Mastitis page) before assuming it is simple engorgement.
General Acupuncture Points

Acupuncture points traditionally used for breast engorgement — 12 key points, used across the patterns below rather than tied to just one.

PointKey action
LR-3Subdues Liver Yang, primary point to move stagnant Liver Qi.
LR-14Alarm point of the Liver, directly unbinds Qi in chest and breast.
LR-2Clears Liver fire, cools Heat, calms explosive anger.
ST-18Local point on the breast, directly unblocks Qi and reduces distension.
REN-17Master point for chest Qi, opens the chest and relieves fullness.
ST-40Master point for Phlegm, transforms and drains Phlegm-Dampness.

The Six Patterns

1. Liver Qi Stagnation View full pattern →

Clinical manifestations: Distension or bloating along ribs and flanks, breast distension with tenderness and swelling before periods, frequent sighing, irritability or short temper.
Tongue: Normal or slightly dark body with red or purple sides. Pulse: Wiry, tight like a guitar string, most prominent at left middle.
Treatment principle: Course the Liver and resolve constraint, invigorate Blood and dispel stasis.
Note: no moxa. Direct match on the core distension/irritability/sighing picture.

2. Liver Qi Stagnation that Transforms into Heat View full pattern →

Clinical manifestations: Breast distension with heat, redness, and tenderness, bitter taste in mouth, irritability or short temper, distending or burning pain along ribs, red face and eyes.
Tongue: Red body with redder sides and yellow coating. Pulse: Wiry and rapid, strongest at left middle.
Treatment principle: Soothe the Liver, clear Heat, relieve breast distension.
Note: no moxa. Direct match with this site's own Stagnant-Liver-Qi-turning-into-Heat slide.

3. Phlegm-Dampness in the Middle-Burner

Clinical manifestations: Heavy, distended sensation rather than sharp pain, bloating and fullness in upper abdomen after eating, nausea, poor appetite, loose or sticky stools.
Tongue: Pale, swollen with teeth marks and thick white greasy coating. Pulse: Slippery and soggy, especially at right middle.
Treatment principle: Dry Dampness and transform Phlegm, regulate Qi and harmonize the Middle Burner.
Note: no moxa except tonifying points. No deck on this site has a Middle-Burner-specific Phlegm-Dampness slide, so no pattern link is given here.

4. Kidney and Liver Yin Deficiency View full pattern →

Clinical manifestations: Chronic dull breast distension, lower back and knee soreness, night sweats and heat in palms/soles, dry eyes and blurred vision, dizziness and tinnitus.
Tongue: Red, thin body with cracks, little or no coating, dry. Pulse: Fine, rapid, often wiry, weak at left Chi.
Treatment principle: Nourish Liver and Kidney Yin.
Note: no moxa. Direct match, reusing the well-established Kidney-and-Liver-Yin-deficiency slide.

5. Qi and Blood Deficiency

Clinical manifestations: Chronic dull breast distension, fatigue and weakness especially after exertion, pale complexion and lips, poor appetite.
Tongue: Pale body, may be thin or teeth-marked, thin white coating. Pulse: Fine and weak, especially at right Guan.
Treatment principle: Tonify Qi and Blood, strengthen the Spleen.
Note: moxa applicable. No deck on this site combines Qi and Blood deficiency into one slide, so no pattern link is given here.

6. Rebellious Liver Qi View full pattern →

Clinical manifestations: Heavy, distended sensation rather than sharp pain, belching, nausea, poor appetite, loose or sticky stools, distension along ribs and flanks, frequent sighing.
Tongue: Normal or slightly red on sides, thin white coating. Pulse: Wiry, especially at left middle.
Treatment principle: Smooth Liver Qi and encourage downward flow rather than rebellious upward surge.
Note: no moxa. Direct match — the deck slide names breast distension explicitly among its symptoms, first use of this specific slide.
