Chronic Pyelonephritis

How Chinese medicine differentiates chronic pyelonephritis into the five patterns you'll actually meet in practice, and the points for each. Distinct from this site's own Urinary Tract Infection and Chronic Kidney Disease pages. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Fever, flank pain, or worsening kidney function needs prompt medical evaluation — acupuncture here is supportive of the underlying pattern, never a substitute for that care.
General Acupuncture Points

Clinical manifestations: Frequent, urgent urination worsening with fatigue, straining or dribbling urination, deep achy soreness in the lower back and knees, poor appetite, loose unformed stools, pale sallow complexion.
Tongue: Pale and swollen with tooth marks along the edges, white moist coating. Pulse: Deep, weak, and fine.
Treatment principle: Tonify Spleen and Kidney Qi, raise sunken Yang, lift sinking Qi.
Note: moxa applicable throughout. Linked to this site's own Kidney-and-Spleen-Yang-deficiency slide, the closest match, with the plain-Qi-deficiency framing here flagged against that slide's Yang-deficiency framing.

2. Kidney Yin Deficiency View full pattern →

Clinical manifestations: Scanty, dark yellow urine, burning sensation during urination, night sweats and five-center Heat, lower back soreness and weakness, dry mouth and throat worse at night.
Tongue: Red, thin body with little or no coating, dry surface with possible cracks. Pulse: Fine and rapid, weak at the left Chi position.
Treatment principle: Nourish Kidney-Yin, clear deficiency Heat, generate fluids.
Note: no moxa. Direct exact-name match with this site's own Kidney-Yin-deficiency slide.

3. Kidney Yang Deficiency View full pattern →

Clinical manifestations: Dribbling or hesitant urination improved by warmth, cold aching pain in the lower back and knees, multiple nighttime urination, cold hands and feet, intolerance of cold.
Tongue: Pale, puffy, tender body with teeth marks, wet white coating. Pulse: Deep, slow, and weak, especially at the Chi position.

Tongue: Red body with yellow greasy coating, thickest at the back. Pulse: Slippery and rapid, pronounced at the Chi position.
Treatment principle: Clear Heat and drain Fire, promote urination and relieve stranguria, clear Damp-Heat from the Lower Burner.
Note: no moxa; foul-smelling urine with heaviness needs medical evaluation regardless (possible active infection). Linked to this site's own Damp-Heat-in-the-Bladder slide via the established Lower-Burner organ-system convention.

5. Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing pain in the lower abdomen or pelvis, pain worsens with pressure and at night, dark menstrual blood with clots, rough dry skin or dark circles under the eyes.
