Mania

How Chinese medicine differentiates manic states into the four patterns you'll actually meet in practice, and the points for each. Distinct from this site's Bipolar Disorder and Schizophrenia pages. Acupuncture only β€” no herbs.

This page is educational, for practitioners and students β€” it is not a diagnostic tool and does not replace a proper in-person assessment. Acute mania, especially with risk of harm to self or others, needs immediate psychiatric care. Acupuncture is a complement to psychiatric treatment, never a replacement.
General Acupuncture Points

Acupuncture points traditionally used for mania β€” 3 key points, used across the patterns below rather than tied to just one.

PointKey action
HE-7Calms the Mind and opens the Mind’s orifices. Nourishes Heart-Blood.
P-6Opens the chest and moves Qi and Blood. Calms the Mind. Moves Liver-Qi.
LIV-3Subdues Liver-Yang. Extinguishes interior Wind. Promotes the smooth flow of Liver-Qi.
The Four Patterns

1. Phlegm-Fire Harassing the Heart View full pattern →

Clinical manifestations: Mental restlessness with incoherent speech; copious thick yellow phlegm rattling in the throat; chest oppression with internal heat; flushed red face and eyes; severe insomnia; bitter taste and thirst.
Tongue: Red, swollen, red prickly tip, thick yellow greasy coat. Pulse: Slippery and rapid, full at the left front position, slippery at the right middle position.
Treatment principle: Clears Heat and transforms Phlegm to calm the Heart-Spirit disturbed by this combination of pathogenic factors.
Note: no moxa.

2. Liver Fire Blazing View full pattern →

Clinical manifestations: Intense irritability with explosive anger; a throbbing headache at the temples or vertex; a red face with bloodshot eyes; a bitter taste; loud ringing in the ears; constipation and dark urine.
Tongue: Red with redder sides, dry yellow coat. Pulse: Wiry, rapid, forceful, especially strong at the left middle position.
Treatment principle: Drains excess Liver Fire and purges Heat to settle the upward-rushing flame disturbing the Heart-Spirit.
Note: no moxa.

3. Phlegm Misting the Heart View full pattern →

Clinical manifestations: Mental confusion or dullness; gurgling or rattling throat sounds; excessive saliva or drooling; depression alternating with disorganized agitation; muttering and staring.
Tongue: Pale, swollen, thick white greasy coat, may appear stiff. Pulse: Slippery, good strength, deep and rolling.
Treatment principle: Opens the Heart's orifices and expels Phlegm to restore mental clarity and resolve the trapped spirit.
Note: no moxa.

4. Blood Stagnation with Heat

Clinical manifestations: Fixed, stabbing pain in the head or body; a dark or purplish complexion; lower abdominal distension and constipation; irritability worsening at night; thirst without a desire to swallow.
Tongue: Reddish-purple with stasis spots, thin dry yellow coat. Pulse: Choppy and rapid, often wiry at the left middle position.
Treatment principle: Invigorates Blood and clears Heat to disperse the obstruction preventing Heart-Spirit nourishment and vent trapped Heat.
Note: no moxa. No combined Blood-Stagnation-with-Heat slide exists on this site's decks β€” no pattern link is given here.
