Painful Intercourse (Dyspareunia)

How Chinese medicine differentiates painful intercourse, the five patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Painful intercourse can have many underlying causes (infection, endometriosis, structural issues) that need proper medical evaluation alongside any acupuncture care.

The Five Patterns

1. Liver Qi Stagnation View full pattern →

Clinical manifestations: Spasmodic, distending pain during intercourse; pain worse with emotional stress or frustration; breast tenderness and rib-side distension; irritability, frequent sighing, or mood swings.
Tongue: Normal or slightly dark body, red or slightly purple sides, thin white coating. Pulse: Wiry, taut like a guitar string.
Treatment principle: Smooth Liver-Qi stagnation, relieve emotional tension, ease pelvic spasm.
Note: no moxa. Direct match with Liver-Qi stagnation.

2. Damp-Heat in the Lower Burner View full pattern →

Clinical manifestations: Burning pain during intercourse; a heavy, dragging sensation in the lower abdomen and legs; foul-smelling yellow or greenish vaginal discharge; pain worse with warmth and humidity.
Tongue: Red body, yellow greasy coating thickest at the root, slightly swollen. Pulse: Slippery and rapid, more pronounced at the Chi position.
Treatment principle: Clear Damp-Heat inflammation from the Liver and Gallbladder channels.
Note: no moxa. Direct match with Damp-Heat in the Bladder.

3. Blood Stagnation in the Directing and Penetrating Vessels

Clinical manifestations: Fixed, stabbing deep pelvic pain; dark purple menstrual blood with clots; pain worse with pressure and at night; palpable masses or lumps in the lower abdomen.
Tongue: Purple or dusky body, possible stasis spots, engorged sublingual veins, thin white coat. Pulse: Choppy, often wiry at the left Guan position.
Treatment principle: Warm the uterus and move Blood to relieve fixed stabbing pain.
Note: moxa applicable on Ren-4. No deck on this site has a Chong/Ren-vessel-specific Blood-stasis category, so no pattern link is given here.

4. Kidney Yin Deficiency View full pattern →

Clinical manifestations: Dryness and burning pain with friction; night sweats and hot flushes; a dry mouth and throat, especially at night; soreness and weakness of the lower back and knees; dizziness and ringing in the ears.
Tongue: Red, thin body with little or no coating, dry surface, possible cracks. Pulse: Fine and rapid, especially weak at the left rear position.
Treatment principle: Deeply nourish Kidney-Yin and replenish the body's foundational fluids.
Note: no moxa. Direct match with Kidney-Yin deficiency.

5. Qi and Blood Deficiency

Clinical manifestations: Dull, aching pain during intercourse; post-coital exhaustion and dizziness; a pale complexion and lips; poor appetite; heart palpitations; generalized fatigue and weakness.
Tongue: Pale body, may be thin or have teeth marks, thin white coating. Pulse: Fine and weak, lacking force.
Treatment principle: Tonify both Qi and Blood, addressing the root deficiency behind dull, malnourishment-type pain.
Note: moxa applicable. No deck on this site combines Qi and Blood deficiency into one slide, so no pattern link is given here.
