Perimenopause

How Chinese medicine differentiates the perimenopausal transition — distinct from Menopause and Menopausal Hot Flashes already covered — the six patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Very heavy or unusual bleeding during this transition needs medical evaluation before assuming a simple pattern.
General Acupuncture Points

The Six Patterns

1. Kidney-Yin Deficiency View full pattern →

Clinical manifestations: Hot flashes, night sweats, dry mouth/throat, lower back/knee soreness, flushed cheekbones, vivid dreams, insomnia.
Tongue: Red, thin, dry with possible cracks, little coating. Pulse: Fine and rapid, weak at the left Chi position.
Treatment principle: Nourish Kidney-Yin and clear Empty-Heat.
Note: no moxa given the Yin deficiency. Direct exact-name match with this site's own Kidney-Yin-deficiency slide.

2. Kidney and Liver Yin Deficiency View full pattern →

Clinical manifestations: Dry eyes with blurred vision, irritability, dull aching rib pain, lower back/knee soreness, night sweats, dizziness, tinnitus.
Tongue: Red, thin with cracks, little coating, dry. Pulse: Fine, rapid, wiry.
Treatment principle: Nourish Liver and Kidney Yin while clearing Heat.
Note: no moxa. Direct exact-name match with this site's own Kidney-and-Liver-Yin-deficiency slide.

3. Kidney-Yang Deficiency View full pattern →

Clinical manifestations: Profound coldness especially in the lower back/legs, soreness/weakness in the lower back/knees better with warmth, frequent nighttime urination, fatigue, low spirits, loose stools/early-morning diarrhea, leg/ankle swelling.
Tongue: Pale, puffy, tender with teeth marks, wet white coating. Pulse: Deep, slow, weak, feeble at the Chi position.
Treatment principle: Warm Kidney-Yang and restore the body's Fire.
Note: moxa central to this pattern. Direct exact-name match with this site's own Kidney-Yang-deficiency slide.

4. Deficiency of Both Kidney Yin and Yang

Clinical manifestations: Alternating hot flashes and chills, lower back cold/aching, cold limbs, night sweats, frequent nighttime urination, dry mouth/throat, loose stools, poor memory, fatigue.
Tongue: Pale, puffy-tender with teeth marks and cracks, thin white coat partially peeled. Pulse: Deep, fine, weak, weakest at the Chi positions.
Treatment principle: Tonify both Kidney Yin and Yang to stabilize temperature regulation.
Note: moxa applicable throughout. This site's Kidney deck has no combined Kidney-Yin-and-Yang-deficiency slide despite it being a classic textbook pattern (confirmed absent since Chronic Kidney Disease, batch 12), so no pattern link is given — points still link fine individually.

5. Disharmony Between Heart and Kidneys View full pattern →

Clinical manifestations: Insomnia with difficulty falling asleep/frequent waking, palpitations, irritability, vivid/disturbing dreams, poor memory, five-palm heat, night sweats, lower back/knee soreness, anxiety.
Tongue: Red, thin body with a redder tip, little/no coating, possible midline crack. Pulse: Fine and rapid, weaker at the rear/Chi position, stronger at the front/Cun position.
Treatment principle: Nourish Heart-Yin, anchor Heart-Fire, and restore Heart-Kidney communication.
HE-7 ↑ KI-3 ↑ P-6 ⊥ EX-HN-3 (Yintang) ⊥
Note: no moxa. Direct match with this site's own Kidneys-and-Heart-not-harmonized slide — this pattern's own Yin-deficiency-type presentation (five-palm heat, night sweats) confirms the correct side of the recurring distinction first resolved for Early Awakening and Frequent Night Waking. EX-HN-3 (Yintang) has no dedicated deck page on this site, so it renders as a plain chip.

6. Liver-Qi Stagnation View full pattern →

Clinical manifestations: Emotional irritability or depression, frequent sighing, breast tenderness/swelling, rib distension/pain, throat lump sensation, irregular menstruation, insomnia with difficulty falling asleep.
Tongue: Normal or light red body with red/slightly purple sides, thin white coat. Pulse: Wiry, prominent at the left Guan.
Treatment principle: Course Liver-Qi, resolve constraint, and harmonize emotional flow.
Note: no moxa. Direct exact-name match with this site's own Liver-Qi-stagnation slide.
