Peripheral Neuropathy

How Chinese medicine differentiates peripheral neuropathy as a Painful Obstruction (Bi Syndrome), the six patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. The underlying cause of neuropathy (diabetes, nerve compression, toxin exposure, etc.) needs medical diagnosis and management alongside any acupuncture care.
General Acupuncture Points

Acupuncture points traditionally used for peripheral neuropathy — 12 key points, used across the patterns below rather than tied to just one.

The Six Patterns

1. Wind-Cold-Damp

Clinical manifestations: Pain worse in cold or damp weather; heaviness and aching of the limbs; morning stiffness; numbness or tingling; joint swelling without heat.
Tongue: Pale or slightly swollen body, white greasy coating. Pulse: Moderate or soggy, possibly tight if Cold predominates.
Treatment principle: Dispel Wind-Dampness, tonify Qi, invigorate Blood and unblock the channels.

Note: moxa applicable on the Tonify-Qi point. No deck on this site has a Wind-Cold-Damp Bi-syndrome category, so no pattern link is given here.

2. Painful Obstruction due to Damp-Heat in the Channels

Clinical manifestations: Burning pain; redness and heat in the affected areas; hot swelling; heavy aching limbs; thirst; dark, scanty urine.
Tongue: Red body, yellow greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clear Heat and dry Dampness from the channels.
Note: no moxa. No deck on this site has a Damp-Heat Bi-syndrome category, so no pattern link is given here.

3. Phlegm in the Channels, Joints and Muscles

Clinical manifestations: Fixed stabbing hand or foot pain; stubborn numbness with a heavy, wooden sensation; hard, immobile joint nodules; purplish skin discoloration.
Tongue: Purple body with stasis spots, white greasy coating, distended sublingual veins. Pulse: Wiry and choppy.
Treatment principle: Transform Phlegm, regulate Qi and resolve turbidity.
Note: no moxa. No deck on this site has a Phlegm-in-the-channels Bi-syndrome category, so no pattern link is given here.

4. Blood Stagnation View full pattern →

Clinical manifestations: Fixed, stabbing, needle-like pain; nighttime pain worsening; dark purplish skin or nails; pain worse with pressure.
Tongue: Purple or dusky body with stasis spots, distended dark sublingual veins. Pulse: Choppy, possibly wiry.
Treatment principle: Invigorate Blood and dispel stasis to unblock the channels.
Note: no moxa. Matches Liver-Blood stasis — dark purplish skin/nails and fixed nighttime pain are shared discoloration markers.

5. Painful Obstruction with Liver and Kidney Deficiency

Clinical manifestations: Dull aching joint and limb pain; lower back and knee soreness/weakness; numbness or tingling; cold/damp weather aggravation; fatigue; weak heavy legs; pale complexion.
Tongue: Pale, thin body, thin white coating. Pulse: Fine, weak, slow, especially weak at the rear position.
Treatment principle: Dispel Wind-Dampness, relieve the obstruction, and supplement Liver and Kidneys.

Note: moxa applicable on the tonifying points. No deck on this site combines Liver and Kidney deficiency into a single Bi-syndrome slide, so no pattern link is given here.

6. Painful Obstruction with Qi and Blood Deficiency

Clinical manifestations: Dull aching pain worsening after exertion; numbness or tingling in the hands and feet; pale lips and complexion; general fatigue; cold sensitivity; spontaneous sweating.
Tongue: Pale body, possibly thin with teeth marks, thin white coating. Pulse: Fine, weak, possibly slowed.
Treatment principle: Tonify Qi, warm the channels, and nourish Blood to dispel the obstruction.
Note: moxa applicable. No deck on this site combines Qi and Blood deficiency into one slide, so no pattern link is given here.
