Polymyalgia Rheumatica

How Chinese medicine differentiates polymyalgia rheumatica — distinct from Rheumatoid Arthritis, Fibromyalgia and Muscle Tension already covered — the three patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs, used as supportive care alongside medical management, never as a substitute for it.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Polymyalgia rheumatica can be associated with giant cell arteritis, a serious inflammation of the blood vessels — any new headache, vision changes, or jaw pain alongside this condition needs prompt medical evaluation.
General Acupuncture Points

Acupuncture points traditionally used for polymyalgia rheumatica — 8 key points, used across the patterns below rather than tied to just one.

PointKey action
L.I.-11Clears Heat, cools the Blood.
G.B.-34Resolves Liver-Qi stagnation, resolves Damp-Heat.
SP-6Tonifies Spleen and Stomach, resolves Dampness.

ST-36Tonifies Qi and Blood, strengthens Spleen and Stomach.
KI-3Tonifies Kidney Essence, strengthens bones.

The Three Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Painful Obstruction Due to Damp-Heat in Channels

Clinical manifestations: Deep, heavy aching in the shoulders, neck, and hips, muscles feel hot and swollen, stiffness worse in the morning and with heat, low-grade fever or body heat sensation, thirst with desire for cold drinks, dark scanty urine.
Tongue: Red body, possibly swollen, yellow greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clear Heat and drain Dampness, unblock the channels and collaterals, relieve painful obstruction.
Note: no moxa. This is a joint/muscle-localized Damp-Heat Bi-syndrome with no organ marker, the same joint-localized gap noted for Jaw Pain in this same batch and Sinew Contracture/Upper Back Pain in batch 41 — no pattern link is given here; points still link fine individually.

2. Painful Obstruction With Qi and Blood Deficiency

Clinical manifestations: Dull, aching muscle pain worsening with activity, profound fatigue and lack of strength, pale face/lips/nail beds, numbness or tingling in the limbs, sensitivity to cold, spontaneous sweating, poor appetite and shortness of breath.
Tongue: Pale body, thin white coating, possibly teeth marks. Pulse: Fine and weak, possibly slow.
Treatment principle: Tonify Qi, nourish Blood, strengthen the Spleen.
Note: moxa applicable throughout. This is the generic Qi-and-Blood-deficiency picture, already confirmed absent as a standalone slide on this site many times over — no pattern link is given; points still link fine individually.

3. Painful Obstruction With Liver and Kidney Deficiency

Clinical manifestations: Deep, dull aching in the shoulders and hips, lower back and knee soreness and weakness, stiffness worse with cold and damp weather, numbness or tingling, strong preference for warmth, aversion to cold, fatigue with pale or dull complexion.
Tongue: Pale, thin body with thin white coating. Pulse: Fine, weak, slow, especially weak in the rear position.
Treatment principle: Dispel Wind-Dampness, relieve painful obstruction, supplement the Liver and Kidneys.
Note: moxa applicable given the pale tongue and slow pulse. This pattern's own tongue and pulse (pale, slow, weak) point away from Yin-deficiency-type Heat, so it is not forced onto this site's Kidney-and-Liver-Yin-deficiency slide despite the \"Liver and Kidney\" naming overlap — that would misrepresent a deficiency-cold picture as a Yin-deficiency-heat one. No pattern link is given here; points still link fine individually.
