Premature Ejaculation

How Chinese medicine differentiates premature ejaculation, the five patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

The Five Patterns

1. Kidney-Qi Not Firm View full pattern →

Clinical manifestations: Premature ejaculation, often without erotic dreams; soreness and weakness in the lower back and knees; frequent urination with clear, dilute urine; fatigue and a lack of strength; brought on by ageing, chronic illness, or excessive sexual activity depleting Kidney-Qi; eased by rest and warmth.
Tongue: Pale, puffy, moist, thin white coating, possible tooth marks. Pulse: Deep, Weak, Fine, especially feeble at both rear (Chi) positions.
Treatment principle: Tonify Kidney-Qi and astringe Essence to secure the sperm gate.
Note: moxa applicable.

2. Kidney-Yin Deficiency with Empty-Heat Blazing View full pattern →

Clinical manifestations: Night sweats and a hot sensation in the palms, soles, and chest; a malar flush across the cheekbones; mental restlessness and irritability; a dry throat, worse at night; soreness and weakness in the lower back; scanty, dark urine; brought on by chronic overwork, ageing, or a long illness consuming Kidney-Yin; eased by rest and cooling foods.
Tongue: Red, thin, dry, little or no coating, possible cracks. Pulse: Fine, Rapid.
Treatment principle: Nourish Kidney-Yin and clear deficiency Heat to calm the agitated ministerial fire and stabilize the sperm gate.
Note: no moxa.

3. Heart and Spleen Deficiency

Clinical manifestations: Palpitations or an awareness of the heartbeat; poor appetite with bloating after eating; fatigue and physical exhaustion; a sallow or pale complexion; loose or poorly formed stools; insomnia or vivid dreaming; anxiety or being easily startled; brought on by chronic overthinking, long-term stress, or a long illness depleting Heart and Spleen; eased by rest, a calm routine, and nourishing food.
Tongue: Pale, tender, tooth-marked, thin white coating. Pulse: Fine and Weak, especially weak at the left Cun (Heart) and right Guan (Spleen) positions.
Treatment principle: Tonify Qi, nourish Blood, and strengthen the Spleen to anchor the Mind and stabilize the Essence gate.
Note: moxa applicable. The closest deck slide, "Spleen- and Heart-Blood deficiency," matches the general symptom picture closely but has no sexual or ejaculatory content, so no pattern link is given here.

4. Damp-Heat in the Liver Channel View full pattern →

Clinical manifestations: A bitter taste in the mouth; scrotal dampness or itching; dark, scanty, or burning urination; pain or fullness along the ribs; brought on by a diet heavy in greasy, spicy, or alcoholic food, or damp weather; eased by cooling foods and a drier environment.
Tongue: Red with red sides, yellow greasy coating thicker at the centre and root. Pulse: Wiry, Slippery, Rapid.
Treatment principle: Clear Damp-Heat from the Liver channel and lower burner to calm the agitated sperm gate.

5. Liver-Qi Stagnation View full pattern →

Clinical manifestations: Premature ejaculation that worsens with stress or anxiety; distension or bloating along the ribs and flanks; frequent sighing; irritability or mood swings; chest tightness or a feeling of a lump in the throat; brought on by stress, frustration, or suppressed emotion; eased by gentle exercise, emotional release, or a calmer routine.
Tongue: Normal or slightly dark, red or slightly purple sides, thin white coating. Pulse: Wiry, especially at the left middle position.
Treatment principle: Soothe Liver-Qi, resolve constraint, and restore smooth flow through the Liver channel to normalize the sperm gate's function.
