Retained Placenta

How Chinese medicine differentiates retained placenta into the four patterns you'll actually meet in practice, and the points for each. Supportive care alongside emergency obstetric management, not a substitute for it. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Retained placenta is a medical emergency — it needs immediate obstetric management to prevent hemorrhage and infection; acupuncture here is only ever supportive, never a substitute for that care.
General Acupuncture Points

Acupuncture points traditionally used for retained placenta (supportive, alongside emergency obstetric care) — 10 key points, used across the patterns below rather than tied to just one.

PointKey action
SP-4Regulates the Penetrating Vessel. Regulates menstruation.
SP-10Cools Blood. Invigorates Blood and eliminates stasis.
REN-3Resolves Dampness from the Lower Burner. Benefits the Uterus and regulates menstruation.
L.I.-4Removes obstructions from the channel. Harmonizes the ascending and descending of Qi.
REN-4Nourishes Blood and Yin. Strengthens the Uterus and regulates menstruation.
SP-6Strengthens the Spleen. Nourishes Blood and Yin. Regulates the uterus and menstruation.
ST-36Benefits Stomach and Spleen. Tonifies Qi and Blood.
DU-20Raises Yang. Benefits the Brain and the sense organs.
LR-3Subdues Liver-Yang. Promotes the smooth flow of Liver-Qi. Invigorates Blood.
KI-3Tonifies the Kidneys (both Yin and Yang). Benefits Essence. Regulates the uterus.
The Four Patterns

1. Blood Stagnation in the Directing and Penetrating Vessels View full pattern →

Clinical manifestations: Fixed, stabbing lower abdominal pain worsening with pressure, dark purple or blackish lochia with large clots, palpable firm mass or fullness in the lower abdomen, pain improves after passing clots, absence of marked exhaustion or fever.
Tongue: Dark purple body with purplish stasis spots, engorged sublingual veins, thin white coat. Pulse: Choppy and wiry, especially at the left middle position, may be deep at the rear positions.
Treatment principle: Invigorates Blood, dispels stasis, warms the channels, promotes uterine contraction.
Note: no moxa; retained placenta needs immediate obstetric management regardless of pattern. Linked to this site's own Liver-Blood-stasis slide via the established discoloration-marker rule, with the same Chong/Ren-vessel naming nuance flagged as on this site's other postpartum pages.

2. Qi Deficiency Causing Blood Stagnation View full pattern →

Clinical manifestations: Postpartum exhaustion and weakness, weak ineffective uterine contractions, dull heavy abdominal pain that does not move, pale face with a greyish or dusky tinge, shortness of breath and reluctance to speak.
Tongue: Pale body with a purplish tinge, stasis spots, teeth marks, thin white coat. Pulse: Deep, weak, and choppy.
Treatment principle: Tonifies Qi and Blood while gently moving Blood circulation to overcome the sluggishness trapping the placenta.
Note: moxa applicable throughout; needs immediate obstetric management regardless. This is a 2nd, still-within-cap use of this site's own Liver-Blood-stasis slide on this page — the dusky/purplish discoloration markers apply even though this variant is Qi-deficiency-driven, per the established discoloration-marker rule.

3. Heat and Blood Stagnation in the Lower Burner View full pattern →

Clinical manifestations: Fixed sharp lower abdominal pain worsening with pressure, fever worsening at night, dark clotted blood, thick foul-smelling lochia, mental restlessness or agitation, dry mouth and throat.
Tongue: Reddish-purple body with stasis spots, distended sublingual veins, dry yellow coating. Pulse: Deep, choppy, and rapid.
Treatment principle: Clears Heat from the Lower Burner while breaking Blood stasis to soften and expel the retained placenta.
Note: no moxa; fever with foul lochia needs urgent medical evaluation regardless (possible postpartum infection). Rather than a 3rd use of the Liver-Blood-stasis slide (over this page's two-use cap), linked to this site's own Damp-Heat-in-the-Bladder slide via the established Lower-Burner organ-system convention.

4. Kidney and Liver Yin Deficiency View full pattern →

Clinical manifestations: Dizziness and ringing in the ears, dry eyes with blurred or cloudy vision, night sweats and heat in the palms, soles, and chest, soreness and weakness of the lower back and knees, insomnia or restless sleep, weak contractions from insufficient Essence rather than forceful stasis.
Tongue: Red with little coating, possible cracks, dry surface. Pulse: Fine, rapid, and wiry, weak at the left Chi (Kidney) position.
Treatment principle: Nourishes Kidney and Liver Yin to replenish uterine Essence and restore contractile function, gently promotes placental expulsion.
Note: no moxa. Direct exact-name match with this site's own Kidney-and-Liver-Yin-deficiency slide.
