Upper Abdominal Pulsation

How Chinese medicine differentiates a pulsating sensation in the upper abdomen into the five patterns described for it, and the points for each. Distinct from this site's Palpitations and Epigastric Pain pages. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. A visible, strong or new pulsating mass in the abdomen — especially with pain, back pain or fainting — needs urgent medical assessment (aortic aneurysm must be excluded).
General Acupuncture Points

Acupuncture points traditionally used for upper abdominal pulsation — 3 key points, used across the patterns below rather than tied to just one.

PointKey action
REN-12Tonifies Stomach and Spleen. Resolves Dampness and Phlegm. Regulates Stomach-Qi.
ST-36Benefits Stomach and Spleen. Tonifies Qi and Blood. Tonifies the Original Qi.
P-6Opens the chest and moves Qi and Blood. Calms the Mind. Moves Liver-Qi.
The Five Patterns

↑ reinforcing   ↓ reducing   ⊥ even method.

1. Phlegm-Fluids in the Stomach and Small Intestine

Clinical manifestations: Gurgling or splashing sounds; vomiting clear fluid; no thirst; epigastric pulsation with fullness; preference for warmth; dizziness and heaviness.
Tongue: Pale, swollen, tooth-marked, white slippery coat. Pulse: Deep, wiry and slippery.
Treatment principle: Warms Yang and transforms fluid retention.
Note: moxa is appropriate. This site's decks have no Phlegm-Fluids-in-the-Stomach slide — no pattern link is given here.

2. Spleen Deficiency with Dampness

Clinical manifestations: Heavy, dull pulsation worse after eating; abdominal bloating; loose stools; body heaviness like lead.
Tongue: Pale, swollen, scalloped, white greasy coat. Pulse: Soggy and slow.
Treatment principle: Tonifies Qi, strengthens the Spleen and drains Dampness.
Note: Mixed Deficiency/Excess. No deck slide combines Spleen deficiency with Dampness — no pattern link is given here.

3. Water Qi Intimidating the Heart View full pattern →

Clinical manifestations: Epigastric or periumbilical pulsation worse lying down; cold hands and feet; leg and ankle swelling; a sensation of rushing upward; palpitations with night-time anxiety.
Tongue: Pale, swollen, tender, tooth-marked, white slippery coat. Pulse: Wiry and slippery, or deep and fine.
Treatment principle: Warms Yang, disperses Cold and transforms Water-Dampness.
Note: moxa is strongly indicated. Linked to this site's own Kidney-Yang-deficiency-with-Water-overflowing slide.

4. Phlegm-Fluids in the Chest and Hypochondrium View full pattern →

Clinical manifestations: Throbbing below the heart; chest and rib pain worse with coughing; one-sided fullness; shortness of breath; inability to lie flat; thin white sputum.
Tongue: Pale, swollen, white slippery or greasy, excessively moist coat. Pulse: Deep and wiry, possibly slippery.
Treatment principle: Dries Dampness, transforms Phlegm and descends Qi.
Note: moxa is appropriate. Linked to this site's Phlegm-Fluids-obstructing-the-Lungs slide, the closest match.

5. Liver Qi Stagnation Turning into Heat View full pattern →

Clinical manifestations: Distending, throbbing pulsation worse with stress, anger and before the period; bitter taste; irritability; rib pain; acid reflux or sour belching.
Tongue: Red, redder sides, yellow coat. Pulse: Wiry and rapid.
Treatment principle: Courses the Liver, resolves constraint and clears Heat.
Note: no moxa.
