Clinical manifestations: Burning or stinging pain during urination; dark yellow or cloudy, turbid urine; a constant urge to urinate with only a few drops passing; a feeling of fullness and pressure in the lower abdomen; brought on by holding urine too long, poor hygiene, dehydration, or an acute infection; eased by drinking plenty of water, urinating frequently, cranberry or cooling fluids, or prompt medical treatment.
Tongue: Red body, yellow greasy coating, thickest at the root. Pulse: Slippery, Rapid.
Treatment principle: Clear Heat, drain Fire, promote urination, relieve painful urination, clear Damp-Heat from the Lower Burner.
Note: no moxa.
Clinical manifestations: Urinary frequency or urgency that worsens with overwork; chronic fatigue and low stamina; soreness and weakness in the lower back and knees; poor appetite with loose stools; nighttime urination; brought on by chronic overwork, a long illness, ageing, or recurrent untreated infections; eased by rest, warm nourishing food, warmth on the lower back, or moxibustion.
Tongue: Pale, swollen body with tooth marks, white moist or slippery coating. Pulse: Deep, Weak, Thready, especially weak at the right Guan and both Chi positions.
Treatment principle: Tonify Spleen and Kidney Qi to stabilize Bladder function and reduce urinary frequency.
Note: moxa applicable.
3. Heat and Blood Stagnation in the Lower Burner
Clinical manifestations: Dark-red or cola-coloured urine; stabbing pain during urination; fixed lower abdominal pain that resists pressure; irritability and mental restlessness; a fever worse at night; a dry mouth and throat; brought on by an unresolved, prolonged infection or an acute severe flare; eased by prompt medical treatment, rest, and plenty of fluids.
Tongue: Reddish-purple body with stasis spots, distended sublingual veins, dry yellow coating. Pulse: Deep, Choppy, Rapid.
Treatment principle: Cool the Blood, stop bleeding, promote urination, relieve painful urination, clear Heat from the Lower Burner.
Note: no moxa. Dark or cola-coloured urine, especially with fever or flank pain, can indicate a kidney infection or another serious condition and needs prompt medical evaluation. No deck slide combines Heat and Blood stagnation in a urinary presentation, so no pattern link is given here.
Clinical manifestations: Scanty, dark urine, possibly appearing light-red; mild burning or discomfort on urination, not intense; symptoms that worsen in the late afternoon and evening; night sweats with heat in the palms, soles and chest; soreness and weakness in the lower back; a dry throat, especially at night; brought on by a long illness, chronic recurrent infections, ageing, or lack of sleep; eased by rest, cooling hydrating food, avoiding overheating, or a calm evening routine.
Tongue: Red, thin, dry body, little or no coating, possible cracks. Pulse: Fine, Rapid, especially weak at the rear position.
Treatment principle: Nourish Yin, clear deficiency Heat, address the underlying depletion driving the urinary symptoms.
Note: no moxa (Yin deficiency).
5. Qi Stagnation
Clinical manifestations: Urinary hesitation or an interrupted stream; distension or bloating in the lower abdomen; symptoms that worsen with emotional stress; frequent sighing or irritability; brought on by stress and frustration, suppressed emotions, or a sedentary lifestyle; eased by gentle exercise, deep breathing, talking through emotions, or a calm environment.
Tongue: Normal or slightly dusky body, thin white coating. Pulse: Wiry, especially at the left middle position.
Treatment principle: Move stagnant Qi downward, unblock the urinary passage, restore smooth Qi flow.
