Foul-Smelling Urine

How Chinese medicine differentiates foul-smelling urine — distinct from Cloudy Urine and Dark Urine already covered, which focus on clarity and colour rather than odour — the four patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Foul-smelling urine with fever, pain, or blood needs medical evaluation before assuming a simple pattern.

The Four Patterns

1. Damp-Heat in the Bladder View full pattern →

Clinical manifestations: Burning or stinging pain during urination, dark yellow, cloudy or turbid urine, frequent urgent need to urinate, lower abdominal fullness or pain.
Tongue: Red body with yellow, greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clear Heat and drain Dampness from the Bladder, promote urination.
Note: no moxa. Direct exact-name match with this site's own Damp-Heat-in-the-Bladder slide.

2. Damp-Heat in Stomach and Spleen

Clinical manifestations: Epigastric bloating and fullness, poor appetite, sticky incomplete bowel movements, heavy foggy sensation in the head and body, dark yellow turbid urine without burning pain.
Tongue: Red body with swollen appearance, thick yellow greasy coating. Pulse: Soggy and rapid.
Treatment principle: Clear Heat and drain Dampness from the digestive system, regulate Qi through the Middle Burner.
Note: this site's decks have no combined Damp-Heat-in-Stomach-and-Spleen slide — now the single most-recurring absence across this whole project, confirmed again here — so no pattern link is given; points still link fine individually.

3. Cold-Dampness in the Lower Burner View full pattern →

Clinical manifestations: Cloudy, pale, turbid urine, stale/musty/fishy odour, cold heavy sensation in the lower abdomen, dull lower back pain worse with cold, frequent copious urination, cold limbs and fatigue.
Tongue: Pale, swollen body with teeth marks, thick white greasy coating. Pulse: Deep, slow, soggy.
Treatment principle: Warm Kidney-Yang, drain Dampness, separate clear from turbid fluids.
Note: moxa applicable throughout. Matched to this site's own Damp-Cold-in-the-Bladder slide — Cold-Dampness and Damp-Cold are the same clinical entity, and the source's own cold-limbs/cold-lower-back/cold-urine picture matches it directly.

4. Kidney-Yin Deficiency With Empty-Heat Blazing View full pattern →

Clinical manifestations: Scanty dark urine with strong smell, night sweats and five-palm heat, dry mouth and throat especially at night, malar flush and low-grade afternoon heat, lower back soreness, mental restlessness or insomnia.
Tongue: Red, thin, dry body with little coating. Pulse: Fine and rapid.
Treatment principle: Nourish Kidney-Yin, clear deficiency Heat, anchor floating Yang.
Note: no moxa given the Empty-Heat. Direct match with this site's own Kidney-Yin-deficiency-Empty-Heat-blazing slide.
