Hematuria (Blood in the Urine)

How Chinese medicine differentiates blood in the urine — distinct from Cloudy Urine and UTI already covered — the five patterns you'll actually meet in practice, and the points for each. Acupuncture only — no herbs.

This page is educational, for practitioners and students — it is not a diagnostic tool and does not replace a proper in-person assessment. Blood in the urine always needs prompt medical evaluation (infection, stones, and — especially with painless bleeding — more serious causes must be ruled out) before assuming a simple pattern.

The Five Patterns

1. Qi and Yin Deficiency

Clinical manifestations: Fatigue, shortness of breath on exertion, dry mouth with little desire to drink, blood in the urine worsens with fatigue, warm palms and soles.
Tongue: Red or pale-red body with thin/absent coating. Pulse: Fine and weak, possibly rapid.
Treatment principle: Tonify Qi and nourish Yin so Qi can contain the Blood and the mild deficiency Heat is cleared.
Note: moxa applicable, light technique given the Yin component. This site's decks have no combined Qi-and-Yin-deficiency slide (confirmed absent again here), so no pattern link is given — points still link fine individually.

2. Kidney-Yin Deficiency With Empty-Heat Blazing View full pattern →

Clinical manifestations: Dark, scanty urine with blood, night sweats, five-palm heat, lower back soreness, dry throat at night, irritability.
Tongue: Red, thin, dry with little or no coating. Pulse: Fine and rapid, weak at the rear position.
Treatment principle: Nourish Kidney-Yin and clear deficiency Heat before it scorches the delicate Lower-Burner vessels.
Note: no moxa given the Empty-Heat. Direct match with this site's own Kidney-Yin-deficiency-Empty-Heat-blazing slide.

3. Kidney-Qi Not Firm View full pattern →

Clinical manifestations: Blood in the urine worsens with fatigue or prolonged standing, sore and weak lower back and knees, frequent clear urination at night, fatigue.
Tongue: Pale, puffy, moist body with thin white coat and tooth marks. Pulse: Deep, weak, fine.
Treatment principle: Tonify Kidney-Qi so it can hold urine and Blood inside the vessels.
Note: moxa applicable throughout. Direct exact-name match with this site's own Kidney-Qi-not-Firm slide — the fatigue-worsened bleeding and frequent clear nighttime urination are exactly the presentation that slide covers.

4. Spleen Not Controlling Blood View full pattern →

Clinical manifestations: Mild or microscopic blood in the urine, fatigue, pale complexion, poor appetite and loose stools, easy bruising or prolonged bleeding.
Tongue: Pale, puffy, tender with teeth marks, thin white coating. Pulse: Fine and weak.
Treatment principle: Tonify Spleen-Qi to restore its Blood-holding function.
Note: moxa applicable throughout. Direct exact-name match with this site's own Spleen-not-controlling-Blood slide.

5. Damp-Heat in the Bladder View full pattern →

Clinical manifestations: Burning or stinging pain during urination, dark, cloudy urine with blood, lower abdominal fullness, sudden onset or acute flare-ups.
Tongue: Red body with yellow, greasy coating. Pulse: Slippery and rapid.
Treatment principle: Clear Damp-Heat so it stops agitating the Blood and damaging the delicate Lower-Burner vessels.
Note: no moxa. Direct exact-name match with this site's own Damp-Heat-in-the-Bladder slide.
